1 DD form 1172 -2 for Dependents or PCS Orders DD1172-2 for Dependents : A DD1172-2 must be signed by the sponsor prior to issuance of a dependent ID. If you have a Common Access Card (CAC), you may sign, view or print this form online at If a sponsor is unable to sign the DD1172-2 online, they may come to the ID card office to sign one in person. This will be scanned into RAPIDS and is valid for 90 days from the date of signature. Please note that a DD1172-2 that is not generated by a RAPIDS ID card office or signed electronically through the DMDC Self-Service website will not be accepted.
