2 All students submitting an application packet will receive e-mail notification through the SCC Student E-Mail Account indicating their admissions status by mid-June 2018. Those accepted into the Program will receive additional information regarding mandatory orientation The criminal background investigation and drug testing sheet is for your information only. Upon acceptance into the Program you will receive more information on the process for completing the criminal background investigation and drug testing as well as registering for Fall Term classes during the mandatory Program Orientation scheduled during Summer Term. 3 Omission of any of the following items may cause your packet to be deemed incomplete. _____ 1. The official Medical Laboratory Technology Program selective ranking score sheet. This score sheet must be completed in its entirety neatly and legibly in ink. 2. A printed GPA calculator to show a grade point of or higher. Use only the GPA calculator accessed through the Medical Laboratory Technology Program website. 3. Transcripts of all previous college course work submitted for consideration to support the grades reflected on the official score sheet. (For courses in which you received transfer credit, you must submit a Transfer Equivalency Report issued by Spartanburg Community College Records Office identifying the courses for which you received credit and the grade earned.) **You do not need to submit a high school transcript.

4 It should be on file as part of your admissions process to Spartanburg Community College. Program faculty will verify points for high school courses. You may want to verify that a copy is on file. 4. Copy/proof of any previous academic degrees earned to support the points reflected on the official score sheet. (Copy of actual degree or copy of official transcript listing degree/date awarded.) 5. Proof of employment in a Health Related Professions site to include date of hire, length of employment, and job capacity to support the points reflected on the official score sheet, OR proof of volunteer work to include agency, dates of volunteer work and volunteer position capacity. This should be from the supervisor on company letterhead. You do not need to return this checklist. It is for your benefit only. (10/4/2017) Official Forms 2018 CRIMINAL BACKGROUND INVESTIGATIONS (CBI)/DRUG TESTING POLICY HEALTH AND HUMAN SERVICES DIVISION SPARTANBURG COMMUNITY COLLEGE A criminal background investigation (CBI) and drug testing are required for each Health and Human Services student who has been accepted into a curriculum Program of study with the exception of ITP and ECD (only CBI required).

5 Students who have been found guilty, by a court of law, or pled no contest (nolo contendere) to a crime, when conviction has occurred within the last 7-10 years, of the following crimes are deemed unqualified to attend clinical training. Crimes include, but not limited to the following: 1. Child or adult abuse 2. Sexual assault 3. Assault with a deadly weapon 4. Neglect 5. Mistreatment of residents, patients/clients 6. Misappropriation of resident/patient/client property The clinical affiliate may exercise discretion regarding other convictions. A criminal background check must be completed for each state the student has been a resident in the last 12 months. Both the criminal background check and drug test will only be conducted at the students expense after he/she has been accepted into a curriculum Program of study. The results of the background check and drug test must be obtained before the student will be allowed into the clinical site.

6 If the results of either of these procedures document a violation as indicated above, the results will be sent to the clinical facility to be reviewed and a determination of whether the student will or will not be allowed in clinical by that facility. The healthcare facility will send in writing a statement whether the student has approval to attend clinical rotations within their facility. A student with a positive drug test will be dismissed from the Program for 1 year. This will count as one Program attempt. He or she may recycle into a Program only once. Upon readmission to the Program , drug testing at the student s expense will be required each semester until he or she completes the Program . Any student unable to attend a clinical affiliate will be required to withdraw from his or her Program of study. If a student has a documented offense after completing the CBI check, he/she must notify the department head or Program coordinator in writing within 24 hours of the offense occurring.

7 If the department head or Program coordinator is not appropriately notified in writing within the time limit, disciplinary action will be taken against the student if this information has been documented in the public domain. *This form is for your information only do not return it in the application packet.
