Delores A. Auzenne Fellowship for Graduate Study

1 Fellowship INFORMATIONThe Delores A. Auzenne Fellowship is designed to assist Florida's State Universities in increasing the enrollment of students from underrepresented groups to Graduate programs. Competitive applicants will be academically talented and contribute ethnic or gender diversity to specific UNF Graduate programs. For example: In fields where males are underrepresented ( Nursing and Special Education), consideration will be given to male be eligible, students must meet the following been accepted into select Graduate disciplines and professional programs where minoritygraduate students are disproportionately under-represented; demonstrated initiative in pursuing educational goals and possess the qualifications toassure satisfactory progress toward future educational goals; either citizens or permanent resident aliens (a copy of your permanent resident alien cardmust be provided) of the United States.

Auzenne Fellowship is a state-funded scholarship offered under the following conditions:1. The amount of this Fellowship is $ each academic year divided equally between Fall and Spring terms. TheFellowship is renewable for one year under the criteria outlined below and will be funded no more than four (4)semesters for a master's degree student and eight (8) semesters for a doctoral student, or until a master's or doctoraldegree is received, whichever occurs In order to be eligible to receive this Fellowship and be eligible for renewal, awardees must agree to the following:(Please check each box in acknowledgement that you have read it.)

6 I must continuously enroll as a full-time Graduate student as defined by my program of Study during the Fall and Spring semesters in the program of Study to which I was admitted. I must successfully complete 18 semester hours as a master s student or 12 semester hours as a doctoral student each academic year (Fall and Spring semesters) at UNF. Hours taken at another college or university will not be counted towards satisfying required hours. I must agree to comply with all University regulations, rules and codes of conduct governing students on campus and remain in good academic standing as a I am awarded a combination of federal, state, private, foundation, and/or institutional financial aid and fellowships which exceeds the Cost of Education as established by the UNF Office of Financial Aid, my award may be reduced until the total value of my aid equals my Cost of Education.

7 My Fellowship will be cancelled if: I do not complete the required semester hours and maintain the required GPA during the Fall and Spring terms, if I break continuous full-time enrollment at UNF, if I am no longer considered a student in good academic standing, or if I fail to fulfill any other conditions of this Auzenne Fellowship Acceptance Agreement. In the event I choose to attend UNF s summer semester, tuition and fees are my own expense. If I completely withdraw from the University of North Florida and receive a tuition refund, I am required to reimburse the University for the full amount of the award as it pertains to the refunded tuition.

8 My award is contingent upon the availability of funding. In the event that I receive an incomplete grade, it is my responsibility to immediately notify the Director of the Graduate School in writing regarding my plans to have the incomplete grade removed. Furthermore, it is my responsibility to provide the Director with timely verification that the incomplete grade has been removed and provide documentation of the final grade. I understand that failure to finish incomplete coursework in a timely manner or otherwise maintain satisfactory academic progress may jeopardize my eligibility for subsequent awards.
