To verify the license or permit of any one of the BOARD s licensees, and view disciplinary documents if applicable, the following steps may be helpful.
2 1. From the homepage, , select License Verification from the subject tabs displayed across the top of the page. 2. Select License Verification (again). 3. Select the license or permit type. 4. On the License Search page, enter either the LAST NAME ONLY or the LICENSE NUMBER ONLY (no letter prefix). 5. If your search was by license number, when you hit enter, the desired licensee s name should be displayed. If by last name, there may be a list from which you will select the desired name. 6. Do a left-click over the name of your licensee, and on the next page, scroll down and select the document you want to view and/or print.
