2 NCO's can participate if they meet the following: a. The NCO must meet all Army Retention Standards. b. The nominee has not received any Article 15s within two years. c. Nominees who do not have direct supervision of Soldiers may submit proof of active participation in external leadership roles. d. Pass Army Physical Fitness Test /Height and Weight standards; and obtain recommendation by Company First sergeant , Battalion Command sergeant Major, and Brigade Command sergeant Major. 1. The sergeant audie murphy Award (SAMA) board will convene on the following dates: Packet Due dates: board Proceedings: 5 December 2016 14 December 2016. 27 February 2017 8 March 2017. 1 June 2017 14 June 2017. 2. Recommend sponsors are SAMC Members, if a SAMC member is not available, supervisors are encouraged to be the sponsor. SAMC Members and candidates will report on the scheduled APFT, meet and greet for packet turn in, and board date.

3 The events of the DIVISION sergeant audie murphy Award Selection board will take place over two days. Before attending the Installation level board , sergeant audie murphy Award Candidates must complete the following SAMA Selection process: 1. AFYB-CSM. SUBJECT: Memorandum of Instruction (MOI) for the FY17 Fort Carson sergeant audie murphy Award Selection board (SAMA). a. Phase One- is Senior NCOs Evaluation/ nomination : Senior NCOs who recommend NCOs for the award of SAMA will screen and evaluate candidates prior to appearance before initial selection board . The recommendation will be presented to the board in the form of a written recommendation highlighting the following: 1) Describe leadership achievements and NCO performance that merits special recognition. 2) How has the NCO contributed to the development of a professional NCO Corps and a combat ready Army?

4 3) How has the NCO exemplified leadership characterized by personal concern for the needs, training, development, and welfare of Soldiers and concern for families of Soldiers? (Include information outlined in phase 2 performance test, APFT and weapon qualification date and unit averages will be included). b. Phase Two- is Performance Test: The SAMA performance test is based on recorded accomplishments of the candidate and their subordinates (when applicable) and must be enclosed with the Senior NCOs evaluation/ nomination . Phase two performance test information will be included in phase one memo. c. Phase Three- is Initial Selection board : An initial selection board will be conducted at brigade level under the direction of the unit's senior NCO. board members will be provided a copy of the candidate's packet. Each candidate's packet will contain at a minimum: 1) Senior NCO's Evaluation/ nomination and the performance test.

5 2) Current DA photo. 3) A biography of the service member. 4) Current Enlisted Records Brief (DA Form 2A, for Army Reserves and Army National Guard Soldiers). d. The board will be comprised of voting members senior to the Candidate and will include at least one voting member of the same sex as the candidate being considered. b. Candidates selected to continue will be scheduled to appear before the final selection board . Requests to appear before the SAMA final selection board will be submitted in a memorandum format to the final selection board president, signed by the brigade level or higher senior NCO. e. Phase Four- is the Final Selection board : SAMA candidates will be considered by a final selection board appointed by the FORSCOM installation Senior CDR's CSM. Four additional CSMs or SGMs will be appointed by the president and a recorder without vote.

6 At least one board member must be of the same sex as the nominee being considered. The final selection board will determine whether or not the candidate has reached a level of knowledge in a range of subject matters to warrant the FORSCOM. The board president has the final approval on all nominees appearing before the board . 3. The sponsor will ensure that the President of the board receives their candidate's Leader's Book prior to the board appearance.

7 Sponsors that are a sergeant audie murphy Award recipient must be in good standing, which is not limited to being current on club membership dues and actively participating within the organization. Additionally, sponsors are not limited to the First Line Supervisor, Platoon sergeant , Company First sergeant , Battalion Command sergeant Major, and Brigade Command sergeant Major. Command sponsorship is highly encouraged; exceptions to sponsors will be granted if the above listed is unavailable. If operations dictate that a sponsor is not available, prior coordination must be made with the final phase SAMC. Representative for the purpose of updating and obtaining approval from the President of the board . The uniform for SAMA candidates and sponsors is the Army Service Uniform; board members will be in Army Combat Uniform. 4 . sergeant audie murphy Award nomination packets will be routed through Headquarters, 4th INFANTRY DIVISION and Fort Carson 6105 Wetzel Avenue, Building 1435.

8 Fort Carson, CO 80913. Packets will be secured with paperclips (no staples) in a manila folder. The upper left-hand corner of the folder will be the Soldier's rank, name and unit. Furthermore, submit a digital packet via email in a legible word document and PDF to or call 719-503-0139/0140. 5. Packets will not be accepted after the turn in suspense date. Packets should be reviewed by a Brigade level CSM and SAMC member for completeness and accuracy. If packets are incomplete this may disqualify candidates and they will not be allowed to continue with the board proceedings. Please use the attachments as a sample product for all submitted documents. Ensure all items on the checklist are complete and accurate; improperly formatted products may not be accepted. CSMs, it is imperative that you check your candidate's packets thoroughly to set them up for success.

9 6 . The board will be held at 4th INFANTRY DIVISION Headquarters, 1st Floor, in room 107. The board president has the final approval on all nominees appearing before the board . The order of appearance will be published at the beginning of the scheduled board . 7. CANDIDATES: a. Candidates must know the sergeant audie murphy biography verbatim. b. Candidates must know the Creed of the Noncommissioned Officer. c. Candidates will possess no derogatory actions in his/her record within the past two years. 3. AFYB-CSM. Candidates participating in phase one through four board proceedings must be administered an Army Physical Fitness Test/Height and Weight Screening on the day of the board .

10 During phase three simultaneous events are highly encouraged during the board proceedings, examples include weapons familiarization and assembly/disassembly of individual assigned weapons and crew served weapons, properly loading a SKL/FM; the above mentioned are example expectations during board proceedings conducted on the day of the scheduled board . The final phase board will consist of in depth situational/scenario driven questions with no limitations with time for evaluation. f. Candidates will be administered a record APFT from the final phase board on the day of the scheduled board . If the NCO does not pass the APFT then the NCO. will not be authorized to participate in board proceedings. Previous APFT. administered at the Brigade Level date is limited to 30 days for the packet submission to the final phase SAMA Selection board . Candidates must be able to perform a three event APFT (pushups, sit ups, and 2 mile run) on the day of the APFT.
