1 Directives and Forms Letter Transmittal Catalog Publication 223 December 2017. 1. Purpose. This Catalog lists all national Postal Service Directives and Forms , as well as government Forms and documents used by the Postal Service. This is a complete revision of the September 2017 edition. Revisions appear regularly in the Postal Bulletin under Directives and Forms Update. This issue contains all of the revisions published in the Postal Bulletin through January 4, 2018. 2. Directives and Forms Management. The policies and procedures for originating, developing, revising, and managing policy and procedure documents are contained in: Management Instruction (MI) AS-310-2013-6, Management of Policy and Procedure Information.

Directives are documents that contain statements of policy or procedure meant to guide Headquarters, field organizations, or the public. They make available the types of information the Postal Service needs to manage for legal, contractual, and business purposes; and include handbooks, headquarters circulars, kits, labels, management instructions, manuals, notices, the Postal Bulletin, posters, publications, signs, and tags.

7 Chapter 2 lists Directives numerically by document identifier. There is also a list of Directives that are free to the public. Postal Service Forms (Chapter 3). Postal Service Forms are documents both printed and electronic used for collecting, sorting, transmitting, and recording data. Chapter 3 lists all Forms numerically by Postal Service Identification number (PSIN). This Catalog does not list Forms designed by field units. International Documents (Chapter 4). Chapter 4 consolidates international postal documents into a list cross-referenced by Postal Service Identification number (PSIN) and Universal Postal Union number .

8 Miscellaneous Postal Service and Non-Postal Government Documents (Chapter 5). USPS Documents Chapter 5 lists some miscellaneous Postal Service documents that are not Directives such as the Comprehensive Statement on Postal Operations. Non-Postal Government Documents Chapter 5 also lists non-postal government documents that are widely used in the Postal Service. These documents usually have an identifier that shows their source, for example, IRS (Internal Revenue Service) form 3435. Publication 223, December 2017 1. 9 Chapter 6 lists all documents alphabetically by title that are listed in Chapters 2 5. Subject Matter Index (Chapter 7). Chapter 7 is the subject matter index of all documents listed in Chapters 2 5. How to Find What You Need Postal Service Identification number (PSIN). If you know the Postal Service Identification number (PSIN) of the document you need (for example, HBK AS-805 or PS form 3971), look it up in Chapters 2, 3, or 4. Title If you know only the title of the document, look it up in Chapter 6, Alphabetical Listing of Documents. Subject Index If you are investigating a topic but do not know the specific document you need, look up a key word in Chapter 7, Subject Matter Index.

10 Example: A Postal Service employee who is relocating and is not sure about relocation procedures would look up relocation in the index and find the keyword guidelines, which is cross-referenced to HBK F-12, Relocation Policy. The employee would also find the keyword real estate expenses, . under Claims which is cross-referenced to PS form 4877, Reimbursement of Real Estate Expenses Change of Official Station. What the Column Headings Mean Postal Service Identification number (PSIN). The Postal Service Identification number (PSIN) is the document's identifier.
