1 DOCUMENTS REQUIRED / CHECKLIST FOR OCI [Note: All the DOCUMENTS have to be shown in original to the Consular Officer for verification along with their photocopies. 2 Affidavit from the applicant that he has never held an Indian passport [In case of minor applicant, an affidavit from both the parents that the minor has never held an Indian passport]; iv. Registered Marriage Certificate; v. Spouse Passport; vi. Current/ Latest Passports and Registered Marriage Certificate of both parents of the applicant; vii. Birth Certificate of non-Indian origin parent; viii. Passports and birth certificates of the parents of non-Indian origin parent; (may be REQUIRED ) ix. DOCUMENTS (i, iv, v & v) of (a) above in respect of the previously Indian parent. (c) On the basis of grandparent/great-grandparents previous Indian nationality: i. Same as b( i to viii); ii. Remaining DOCUMENTS in the same order linking the applicant to his/her grandparent; great grandparent.

3 (d) On the basis of Spouse Indian nationality /OCI i. Current passport of self; ii. Birth certificate of self; iii. Registered Marriage certificate; iv. Current Passport of spouse; v. OCI of spouse [if applicable]; vi. Undertaking from both husband and wife that their marriage has completed two years and that if they ever decide to part ways in future, they will immediately inform the Indian Mission/Post in writing to cancel the applicant s OCI; vii. Passports and Birth certificates of applicant s parents [may be REQUIRED ]; viii. Passports and Birth Certificates of applicant s grandparents [may be REQUIRED ]. (e) An applicant can also apply on the basis of: i. Nativity Certificate issued by the competent authority in the State concerned and attested by Ministry of External Affairs, New Delhi; or ii.

4 Domicile Certificate issued by the competent authority in the State concerned and attested by Ministry of External Affairs, New Delhi. (f) When the applicant is not in possession of an Indian Passport or Domicile certificate or Nativity certificate, he/she may be REQUIRED to produce DOCUMENTS such as birth certificate, educational certificates, property DOCUMENTS , bank account DOCUMENTS etc. It may be noted that the applicant should furnish as much documentary evidence as possible to strengthen his/her claim of Indian origin. The examples mentioned above are only illustrative and not exhaustive. The crux of the matter is that the DOCUMENTS should cumulatively prove that the applicant or his/her parents or his/her grandparents was/were born in India and lived in India for sufficient time to be treated as persons of Indian origin.

5 (g) The following DOCUMENTS may also be REQUIRED [where applicable]: i. Change of name/appearance affidavit by Consular Section of High Commission of India, Tanzania/Notary Public. ii. For applicants born in India: Birth Certificate from Indian Municipality or Panchayat. iii. Recently issued birth Certificate (Post dated birth certificates) from Municipality/Panchayat etc.: duly certified by the District Magistrate/District Collector of the area with name, stamp, seal and contact telephone numbers with area code for verification. iv. For applicants born in Tanzania: full version Birth Certificate from Registrar of Births. v. Post dated School Certificate/School Leaving Certificate from India should be authenticated by Director of Education/District Magistrate/District Collector of the area with name, stamp, seal and contact telephone numbers with area code for verification.

6 Vi. For any other foreign applicant: All Certificates [birth, marriage, etc.,] must be in full version, attested by the Indian Mission in that country. If in some other language other than English/Hindi, their translation must also be duly attested by the Indian Mission in that country.
