2 Uniforms When working in an area, department, or function with a specific uniform requirement, employees or other workforce members are expected to wear the uniform in accordance with the uniform policy of that department and union guidelines. Fit of Clothing Clothing should fit well and allow for comfortable movement throughout all required work activities with-out compromising safety or professional image. Excessively baggy or tight attire is not considered acceptable in appearance. Shirt length is required to ensure coverage of the torso. Necklines should adequately cover the chest area. Clothing Condition An appropriate appearance requires clothes to be neat, clean, wrinkle-free and in good condition. Faded and/or frayed clothing are not considered professional in appearance. Shoes Shoes must be safe for working conditions. For safety reasons, patient care staff members should wear closed-toe footwear. Clean athletic shoes of an appropriate color are acceptable when the role/work being performed requires it.

Hair should be pulled back or restrained as appropriate to safety in the work area by anyone who provides direct patient care, works with food, or handles sterile equipment. No hats, bandanas, sweat bands, or head gear may be worn unless required for medical, safety, religious reasons or as part of a uniform. Your badge should be worn above the waist facing forward. For direct patient care providers, badges worn around the neck must be on a break-away cord or a badge pull per facility guidelines.

4 If an identification badge becomes lost or damaged, or in the event of a name or title change, you must notify your manager immediately and a new badge will be made. Accessories (Jewelry, Piercings, Buttons, Stickers) Accessories ( , necklaces, bracelets, earrings, other piercings, buttons, pins, stickers) must be kept to a minimum and worn in a way that prevents contamination and does not present a safety hazard. More Jewelry Details MN Dept of Health Fact Sheet for Food Workers Nails Natural and artificial nails should be kept short. Polish if worn, may not be chipped. 5 More Hand Hygiene Policy Fragrances Out of consideration for patients, visitors and staff who may have sensitivities to fragrances, we ask that you refrain from wearing the following: perfume, cologne, after-shave, scented lotions, scented hairspray, personal care scented fragrances, cigarette smoke odor or controllable body odor. See the " " link to the right for more details on scents and odors. More Indoor Air Quality Policy Tattoos and other body art Allina Health permits team members to display tattoos and other body art at work that do not pose a con-flict with the team member s job or environment. Factors that Allina Health will consider when determining whether body art is unacceptable to display include: Offensiveness to patients, customers, vendors or colleagues based on racial, sexual, religious, ethnic or characteristics or attributes of a sensitive or legally protected nature. Exceptions to the policy Facility-wide exceptions to the policy may be granted at the discretion of the President or Executive Vice President of the business unit ( holidays, sports theme day, casual Friday).

6 Allina Health will reasonably accommodate staff members with a disability or with religious beliefs that may make it difficult for the staff member to comply fully with the Dress code unless the accommodation creates an undue hardship. Accommodations related to attire may be difficult in light of Health and safety, Page 3 10 / 31 / 17 7:18 AM security and infection control considerations. Those requesting a workplace attire accommodation based on a disability or religious belief should contact Human Resources. Compliance Leaders are responsible for enforcing this policy and ensuring all staff members meet the Dress code standards. Any staff member who has questions regarding these standards should consult with his/her supervisor/manager. Staff members are expected to comply with the Dress code policy. Noncompliance with this policy may result in being sent home to change. Staff members should not be using items intended for patients ( blankets, robes, gowns) for personal use.

7 In addition, Allina Health laundered surgery scrubs and disposable isolations gowns provided by the hospital can be worn only by employees who are required to do so. 9 Unless specifically approved in your department, no street clothing ( hooded sweatshirts, fleece jackets, scarves) can be worn over your uniform. Short/long sleeve shirts or turtlenecks may be worn under scrub tops for modesty or warmth. Any color may be worn for these shirts. Recommended colors include white, cream, tan, brown, black, gray, light pink, light blue, or an exact match to the scrub top. These under scrub tops must be only one color and non-patterned. Scrub jackets are optional and must match designated scrub color. If scrub jacket is worn as the primary uniform top with a non-scrub top worn underneath, the jacket must be left on and closed during the entire shift. Approved Head Covering For safety, infection control and appearance reasons, a head covering must be clean, a solid color coordinated with the uniform (if applicable), no longer than mid torso front and back and be worn close to the body (not billowing).

10 In restricted and semi-restricted areas the head covering must be tucked into the scrub shirt and covered with a disposable head covering, such as a surgical hood. The disposable head covering must cover the scarf while in the restricted area.
