6 Brewster Parke reserves the right to change, suspend, or terminate without notice any of its plans, policies, or procedures whether they be contained in this Handbook or in other Company documents. These policies supersede any and all prior policies previously issued by Brewster Parke . All employment with Brewster Parke is for no definite period of time and may be terminated at any time, without prior notice, by either Brewster Parke or the Employee . Employee RESPONSIBILITY. Safety can only be achieved through teamwork at Brewster Parke . Each Employee , supervisor and manager must practice safety awareness by thinking defensively, anticipating unsafe situations and reporting unsafe conditions immediately. Please observe the following precautions: 1. Notify your supervisor of any emergency situation. If you are injured or become sick at work, no matter how slightly, you must inform your supervisor immediately.

7 2. The use of alcoholic beverages or illegal drugs substances or the abuse of legal prescription drugs during work hours will not be tolerated. The possession of alcoholic beverages or illegal drug substances on the facility property is forbidden. 3. Use, adjust and repair machines and equipment only if you are trained and qualified. 4. Get help when lifting or transferring residents and with heavy objects. 5. Understand your job fully and follow instructions. If you are not sure of the safe procedure, don't guess, ask your supervisor. 6. Know the locations, contents and use of the first aid and firefighting equipment. 7. Wear personal protective equipment in accordance with the job you are performing. 8. We operate under the policy of right to know . Employees are encouraged to observe warning labels on chemicals and read the material data safety sheet postings. 9. All personnel are required to have the two-step TB testing in their first year of employment and one-step TB testing on the yearly basis each year thereafter.

8 10. Employees are required to use standard precautions. 11. In addition to our standard precautions, hepatitis B virus (HBV) immunizations are available to all employees. January 1, 2014 Page 2. The facility provides you with a safe workplace including a Hazardous Materials Communication program to provide you with exposure and medical information regarding the hazardous chemicals with which you will be working. Complete details of the program may be obtained at any nurse station and in each department. A violation of a safety precaution is in itself an unsafe act. A violation may lead to disciplinary action, up to and including termination. OUR PHILOSOPHY. 9 Employee Handbook YOUR JOB. ACCOMMODATIONS FOR DISABILITIES. If you have a disability that affects your ability to perform your job duties, please contact your supervisor to discuss the appropriate accommodations we can make for you to safely and effectively perform your job. A statement from your physician describing the extent of your disability and limitations will usually be necessary in order for us to make the proper accommodations. ALCOHOL AND DRUG ABUSE. For the protection of all of our staff and customers, all employees are strictly forbidden to use, possess, or be under the influence of alcohol or illegal drugs at any time during the work day or anywhere on the premises of the Company or a customer. Off duty sale or use of illegal drugs, or abuse of prescription drugs, is also prohibited. As a condition of employment, employees may be required to submit to drug or alcohol testing.

10 Brewster Parke reserves the right to search Employee personal effects to determine whether employees are in possession of illegal drugs, alcohol, or other controlled substances. Employees who are convicted of a criminal drug statute must notify their supervisor of the conviction immediately. If you must use a prescription drug, which causes adverse side effects, , drowsiness or impaired reflexes, you must inform your supervisor that you are taking such medication on the advice of a physician. Your supervisor may send you home or temporarily assign you to a different duty if the side effects of the prescription drug may cause a threat to safety or impair work performance. ATTENDANCE/PUNCTUALITY. Brewster Parke needs the combined efforts of all employees to ensure uninterrupted, efficient operation. Absenteeism and tardiness (defined as being one (1) minute late to a scheduled shift). places burdens upon your fellow workers.
