5 ALL DOCUMENTATION REQUESTED IN THESE RULES MUST BE SUBMITTED ELECTRONICALLY, THROUGH COMPANY PORTAL, UNLESS OTHERWISE REQUESTED BY THE CUSTOMER SERVICE. TEAM. ELIGIBILITY. All vehicles of the private passenger type, including vans, or vehicles weighing less than tons (7000 lbs), and not commercially owned, nor used as a public or livery conveyance for passengers, nor delivery service, nor used in the course and /or scope of a business, employment, profession and/or occupation, nor rented to others and owned by an individual or husband and wife, who reside with each other, are eligible. BINDING PROCEDURES. Applications must be completed in full and signed by both the insured and the agent in all applicable places. The name of all resident operator(s) must be shown on the application exactly as shown on their valid Driver's License or FLORIDA Identification Card.

6 Birth dates and driver's license numbers of all persons currently residing at the insured's address must be listed on the application for insurance (whether they drive or not). Additionally, the names of all relatives and other persons living with the applicant and all other resident operators must be included in the appropriate section of the application. Two clear digital pictures and a completed inspection form are to be included if physical damage is written. The policy will be issued based on the information contained in and with the application and on the MVRs, subject to verification with other sources. THIS IS YOUR BINDING AUTHORITY and must be followed completely for your application to be properly bound. All applications and endorsements must be bound in the producing agent's register book and in accordance with FLORIDA Statutes.

7 Every new and renewal application must be bound using the Company's agency portal. As well, all endorsements must be completed on Company's agency portal. All applications and qualified endorsements must be scanned and uploaded to the Company within the same business day as bound. Failure to comply with any GUIDELINES could result in the loss of appointment and/or action by the MGA against your agency, should a claim occur. Power of attorney signatures are not acceptable;. however, upon death of the Named Insured, we will accept signatures from the Executor of the Estate. Violations of these GUIDELINES may result in revocation of your agency privileges and/or exposure to possible claims for error or omissions. MANDATORY INSPECTION AND PHOTOGRAPHS. If Comprehensive/Collision coverage is requested, each vehicle must be photographed at the time coverage is bound.

9 The producer is required to upload two (2) digital photos, clearly showing all four sides of the vehicle. The license plate must be clearly shown in one of the photos (the policy number and last name of the insured should be attached to the upload). The agent must inspect the vehicle(s) and complete the inspection form. EXCEPTION: Photos and inspection form(s) are waived if the vehicle is purchased new and the dealer Bill of Sale or a new Leasing Agreement is submitted with the completed application. A $ inspection fee will be charged by the agent. Note: The FLORIDA Department of Insurance Motor Vehicle Pre-Inspection Form (DI4-507) may be used in place of the form furnished by the Company. While s. mandates Pre- Insurance Vehicle Inspections and requires specific information be collected in certain FLORIDA Counties, we require that those forms must be completed in all counties.

10 Failure to complete the inspection form may result in suspension of Comprehensive/Collision coverage and subsequent notice sent to lienholder or lessor, if applicable. NEW BUSINESS. Application must be completed in full and signed by the applicant and the producing agent; no power of attorney signatures are acceptable. The Application must include full names, birth dates and operators/driver's license, if applicable, of all operators/drivers of the insured vehicles(s) and all persons, 14 years or older, licensed or not, who reside with the named insured(s). Failure to do so will constitute a material misrepresentation. Motor Vehicle Reports may be used to rate new and renewal business. Agent must submit MVRs with all new business. If statements on the application differ from the records of the Department of Motor Vehicles or other outside sources, the proper additional or return premium will be processed without prior notice.
