1 GE Healthcare TiP-EDSM Online Continuing Education CE Credit Submission To The Florida Department of Health **Important Notice- Please Read Carefully The process for submitting of continuing education (CE) credits for some TiP-Ed Online programs to the Florida Department of Health (DOH) has changed. Specific progams may not have a FL DOH approval letter. Please follow the directions below carefully to avoid delay in submitting credit. All TiP-Ed Online programs have been approved by an organization that is recognized by the American Registry of Radiologic Technologist (ARRT) as a Recognized Continuing Education Evaluation Mechnaism (RCEEM). A FL certificate holder can be awarded contact hours for sucessfully completeing programs that fall into this category.
2 For each program, all relevant CE paperwork is found online in the Continuing Education Information tab with the TiP- Ed Online program. ALL PROGRAMS - VIEW THE PROGRAM AND COMPLETE THE CE CREDIT ACTIVITES. 1. Go to the GE Healthcare Learning System (HLS) web site , view the entire program video and complete the program's feedback form and post-test. 2. Upon successful completion of the online CE credit activities, print your certificate from your Learning History. 3 If the CE is only approved by a RCEEM (example=ASRT) the technologist must self-submit the continu- ing education during their renewal phase with Florida (approximately two months three months prior to FL exp. date). NOTE: Do not send Florida materials to GE TiP-Ed Online; we do not report CE credit to this state. The materials will not be forwarded to Florida DOH and you will not be notified. CE Submission for Programs WITH FL DOH Approval Letter If there is a FL DOH Form 374 online in the Related Documents tab (Item Details screen), follow this procedure. You must submit the required paperwork directly to Florida DOH Radiation Control. Do not send Florida materials to GE TiP-Ed Online; we do not report CE credit to this state. 1. Contact the Florida DOH directly at 850-245-4266 to receive your Florida DOH Bureau of Radiation Control Con- tinuing Education Rosters (Scantron Form DH 406). You must identify your site as a GE TiP-Ed Online subscriber. 2. Complete the entire FL DOH Scantron Roster and retain a copy for your site's records.
5 The Presentation Date must be recorded as teh date you actually completed the program, not the date listed on the DOH Form 374. 3. Upon successful completion of the feedback form and post-test on the HLS, print the GE TiP-Ed Online certificate of participation and the DOH Form 374. 4. Mail the Florida DOH Scantron Roster, the GE TiP-Ed Online certificate, and the DOH Form 374 to the address list- ed below. Please include the full address of your facility, and a contact name and phone number with your paper- work. This allows the DOH to contact you in case there are questions or incomplete information.
