1 Healthcare Facility Reportable Events This system is for the electronic submission of Reportable Events unrelated to the Patient Safety Act. ACUTE CARE FACILITIES LONG TERM CARE FACILITIES Hotline # 800-792-9770 Facility Types: General Acute Care Hospitals, Satellite Emergency Department (SED), Psychiatric Hospital, Special Hospital, Dialysis Centers, Ambulatory Surgery Centers For any questions regarding reporting requirements, URL/Access codes or how to submit a report please contact or 800-792-9770. To submit your Reportable event, you do not need a USER ID and password to log into Hippocrates . Each Facility was provided a Facility specific URL and access code to submit their reports. Click on your Facility specific URL and enter the access code when prompted.

Access the generic site for surveys: You will then be asked to enter your three (3) Facility specific codes, then click submit. This will open the form specific to your Facility .
