The service is made up of a team of mental health professionals, staffed by child and adolescent psychiatrists, clinical nurse specialists, psychologists, family therapists and mental health practitioners. There are also staff who specialise in working with Young people with a learning disability, and neurodevelopmental presentations up to 19th birthday.

2 The team can offer short term consultation and intervention to parents / professionals as well as working individually with Young people and their families. The length of support offered is based on the child s and family s needs. We offer urgent same day consultation to professionals worried about a child s risk via our duty system (see ) and all Young people admitted to hospital following deliberate self-harm / overdose receive an urgent assessment the next working day or when they are medically fit. Service Details Location & Hours Child & Adolescent Unit, Birch Hill Hospital Birch Road Rochdale OL12 9QB Tel: 01706 676 000 Fax: 01706 676 001 Clinic and therapy rooms are accessible for people with disabilities. In some circumstances appointments are offered at alternative venues or at home if clinically appropriate. We also have an interpreter service available. The service is open Monday to Friday 9am to 5pm however some clinicians will offer earlier and/or later appointments where necessary.

3 Outside of working hours, in an emergency / crisis, families and Young people can access support and assessments through local Accident and Emergency (A&E) departments at Royal Oldham Hospital and North Manchester General Hospital where there is duty on call psychiatry support available. Eligibility Primarily, HYM HMR provides specialist assessment and treatment in the community and in hospital (where necessary) for children and Young people: Who have complex mental health difficulties and/or Where previous support has not resulted in an improvement and/or Who have mental health difficulties that place themselves or others at risk The service is one of several organisations providing emotional health and wellbeing support to Young people across the borough, and prior to making a referral to HYM you should consider whether an alternative service would be more appropriate (see ). The HYM team are happy to offer consultation and advice prior to any referral being submitted to support decision making and ensure the best outcome for the Young person and family.

4 See section making a referral. In certain circumstances, school / college or home assessments can be offered. Age HYM sees Young people aged between 0 and 18 years that are registered with a GP and reside in the local area. 0-5 years: Early Attachment service. This service works with parents and infants to promote Healthy attachment and bonding in their relationship. 5-16 years: Core HYM Team. Young people are offered 1:1 interventions based on individual needs. Consultation is also offered to referrers, families and partner agencies. 5-19 years: Neurodevelopmental Team. Young people with neurodevelopmental disorders can be seen up to 19 years if appropriate. 16, 17 and 18 year olds: Transition Team. The team only see Young people on the Emotional and Behavioural Dysregulation (EBD) pathway who present with moderate to severe risk taking behaviours. The team offers a range of interventions specific to the needs of the Young person.

5 Consultation, advice and support are also offered. All other Young people aged 16 and above should access the Healthy Minds service via April 2016 Difficulties Some of the difficulties this team can help with for children and Young people up to the age of 18 years, and up to 19 for neurodevelopmental presentations, include: Mood Disorders Self - harm Anxiety disorders Obsessive compulsive disorders Attention Deficit Hyperactivity Disorder (ADHD) Eating disorders Trauma, including Post Traumatic Stress Disorder (PTSD) Psychosis Mental health difficulties in children and Young people with Autistic Spectrum Disorders (ASD) / learning disability & assessment and diagnosis of ASD Tic disorders / Tourette's Syndrome Other Services Where mental health is not the primary concern or a specialist HYM service is not necessary, there are a number of alternative organisations who can be contacted for support and advice.

6 This list is not exhaustive and is in constant change. Many difficulties that children and Young people face are relatively straightforward and amenable to school or locally-based interventions rather than specialised HYM input. Emotional difficulties such as anxiety and depression Does school know more about the problem? Get them to speak to HYM. If low concern type issues, we are likely to ask school nurse, school based professional to carry out an intervention. Bereavement Allow time for grieving process. If the child does appear stuck , ring HYM for advice. Parental separation Changes in behaviours are likely when parents separate/other significant life events. Consider relate Parenting may need to be considered also to help with readjustment and consistency this is needed from both parents (see above). Self-harm Consider who is best placed to make referral/have consultation.

8 Think about the risk being presented. Discuss with HYM. Immediate support available via A&E if high risk. 9 Families Professionals and volunteers If in doubt, please call HYM on 01706 676000 Making a Referral Consultation and Advice Prior to making a referral, professionals are able to seek advice for those Young people where there is some doubt over which service which would be best placed to offer support. GPs There are two HYM duty clinicians available every day, along with a duty manager and a duty Consultant Psychiatrist for GPs to access for advice and consultation. Tel: 01706 676000 Other Professionals Other professionals across HMR are able to use the duty system to contact a member of the HYM team for advice prior to submitting referrals. Tel: 01706 676000 Out of Hours/ Crisis / Emergency Families and Young people can access support and assessments outside of the working hours (9-5) via the GP, and A&E where duty on call psychiatry professionals, are available. The RAID service offer mental health needs and risk assessments following triage to people presenting in the A&E department or urgent care centre and medical wards.

10 Other Services Before making a referral to HYM, please consider whether an alternative service may be more appropriate for the Young person as detailed in section The HYM team are happy to support this decision-making process and offer advice on completing referrals. Required Information A referral form is attached at appendix 1 giving an overview of the information necessary for the HYM team to make decisions on any referrals received. This form should be sent to the team either by fax or post see above for full details. April 2016 It is critical that consent is gained from the family before the referral is made. Who Can Refer Any professional (health care, education, social care) involved with a Young person is able to refer to HYM with the appropriate consent. Referral Routes Referrals are screened daily into the service. Providing the referral contains enough information for a decision to be made, the team will assess the level of need and urgency.
