1 How to fill-out your Russian Online Application form All Russian visa applicants must fill out their Application form Online , print, sign and date it before submitting to VISACONNECTION. Please use the website below for filling out your Russian Online Application form : Please follow these steps to prepare your Application form for Russian Visa: 1. Install Adobe Acrobat Reader to your Computer. 2. Fill out Online Application form saving every page by clicking SAVE. 3. Save Online Application form to your computer. 4. Print the form double-sided, sign/date in black ink Please note that Russian Consulate accepts only original signatures. 5. Attach one colour picture (35X45mm) to the form . 6. Send original Application form with all the requested supporting documents to VisaConnection. 7. Provide valid security answer for Application ID Number to VisaConnection along with your Application . Please, provide the type of visa you are applying for on VisaConnection Client Information Sheet.

2 For each question only enter ONE ANSWER per field, in order to add a second answer please use the option "Add". your Application will be kept for 30 days at the Russian Consulate Online Database. PLEASE NOTE that each time you make the changes to the form , the system is issuing new Application ID number. Security question and answer DO NOT CHANGE. WHEN your Application form FOR Russian VISA IS READY, ATTACH THE form TO your DOCUMENT PACKAGE AND DELIVER TO VISACONNECTION. Once you are on the website please: 1. Select the country where the Embassy, Consular department of the Russian Federation or Russian visa Application center located, where you intend to apply for the Russian visa. Select language (English, French or Russian ). 2. Select security question & answer/numerical code and click SUBMIT. PLEASE SEND your SECURITY ANSWER ALONG WITH your DOCUMENTS. VISA DETAILS SECTION: Nationality - Enter your citizenship according to the passport you will be using for your trip to Russia.

3 If you had USSR or Russian nationality at some time please select "yes" and indicate when and why you lost it - Enter "yes" in this field, if you formerly had USSR or Russian citizenship; if not enter no . If yes, please specify the answer. Enter your surname first name(s), middle name(s) as shown in your passport. Enter your date of birth and place of birth, as shown in your passport If you were born in Russia, select "yes" and specify when and which country you have immigrated to - If you were born in Russia, answer yes and enter the name of the country which you immigrated to and when. PASSPORT DETAILS: Type of passport - If you have regular Canadian BLUE Passport SELECT TOURIST . your passport must be VALID for SIX MONTH after your departure from Russia. Issued by - Enter the name of the authority your passport was issued by according to your passport. VISIT DETAILS: Name of host Travel Company or Inviting Organization - Enter the name of host organisation or institution to be visited and town (according to the invitation) Reference number - Enter your reference number (according to the invitation) IF BUSINESS: INVITATION OR TELEX NUMBER Confirmation number - Enter your confirmation number (according to the invitation) Itinerary (places of visit) Please put the cities that are on your invitation.

4 Name, address and phone of a person or hotel in Russia that you plan to stay with - List all places where you intend to stay. ADDITIONAL INFORMATION List all educational institutions you ever attended, except high schools - If you have ever attended any educational institutions, excluding secondary school, you must list a maximum of two places of education List your last two places of work, excluding the current one - If you have ever changed your place of employment, you must list a maximum of two previous places of work. LAST VISIT DETAILS Have you ever been issued a Russian visa before? If you were issued Russian visa before, please indicate the date when visas were issued. You must list all countries you have visited in the past ten years. You should only enter one visit per field (country and year). Use approximately up to 15 countries. List all countries which have ever issued you a passport. MISCELLANEOUS INFORMATION Answer "yes", if you currently have relatives in Russia.

5 If the answer is yes, you must indicate them. You must only enter the information regarding one relative per field. Date of your visit to the Consulate or Visa Center - Enter your intended date of visit to the Consular department with the intention of document submission to apply for a visa. Enter the date in "dd/mm/yyyy" format.
