Introduction DHA MyChart provides you with online access to your medical record. It can help you participate in your healthcare and communicate with your health care providers. From DHA MyChart, you can: Review summaries of your previous appointments, including issues addressed during each visit, your vital signs, and tests or referrals that were ordered.

3 View your medications, including dosage information and instructions, and request a refill. View new lab results, as well as trends over time. For example, view results for your past cholesterol tests on a graph to see if you're close to meeting your target number. Access your family members' medical records. For example, you can view your child's growth charts, immunization history, and upcoming appointments. This guide provides an overview of many of DHA MyChart's features and how to use them. 2. Connecting to the service To sign up for DHA MyChart, you must be at least 18 years old. Please follow below mentioned steps to activate your DHA MyChart account: Please visit and click on the Login Icon to access the DHA MyChart portal You will be guided to a new page If you have already registered with Dubai Health Authority, then kindly enter your username and password. If this is your first time to access Dubai Health Authority website, then please click on Register With Us to create a new username & password After successful login, you will be guided to DHA Service start page, where it will prompt to set your preference for Individual Home or corporate home page.

4 Kindly set Individual Home as your default page. Document Title: MyChart Version Page 5 of 26 Now you have access to all the eServices provided by Dubai Health Authority. Please click on the DHA MyChart Icon to gain access to DHA MyChart Portal The system will request you to enter your Healthcard number and the date of birth associated with the health card number. Once you provide the proper Healthcard number and Date of birth, the system will send an authentication code to the mobile number associated with the Health card. Once you provide the proper authentication code, you will be guided to the MyChart site. Document Title: MyChart Version Page 12 of 26 Please click on the DHA MyChart Icon to gain access to DHA MyChart Portal Document Title: MyChart Version Page 13 of 26 The system will request you to enter the Health card and Date of Birth Document Title: MyChart Version Page 14 of 26 Once the Healthcard number and Date of birth is keyed in, the system sends the authentication code to the registered mobile number.

7 In case of no associated mobile number or non-UAE number, the system prompts the user to get in touch with the PHC and update the number to a local number. Document Title: MyChart Version Page 15 of 26 Once the proper authentication code is provided, the user will be guided to the Terms & Conditions page of MyChart. On accepting the terms & conditions you will be given access to DHA MyChart Portal Document Title: MyChart Version Page 16 of 26 3. Functionalities and Features Messaging View messages from your clinic You can read any messages sent by your doctor or other clinic staff by going to your Inbox Document Title: MyChart Version Page 17 of 26 (Messaging > Inbox).

8 Click the message you want to read. From here, you can reply to the message or delete it. If you're looking for a specific message, you can enter key words in the Search field on the Inbox page to locate them. Ask your doctor for medical advice If you have a non-urgent medical question, you can send a message to your doctor's staff members. This message is secure, meaning your information stays private as it is sent over the Internet. You might use the Get Medical Advice feature if you're not sure whether you should come in for an appointment, if you need clarification on the dosage of one of your medications or something that was discussed in a recent visit, or if you just want advice about a common illness. 1. Go to Messaging > Get Medical Advice. 2. Click Ask a general medical question. 3. Select a recipient from the list. This list might include doctor with whom you've recently had an office visit, or the general nursing staff at the clinic.

9 4. Select a subject for your message and enter your question. 5. When you are finished, click Send. Someone at your clinic should respond to you within two business days. If you've opted to receive email or text notification for new messages in your DHA MyChart account, you'll receive a message letting you know that the clinic has responded to your request. Document Title: MyChart Version Page 18 of 26 If you want to view a message after you've sent it, you can do so by going to Messaging > Sent Messages. Messages that appear in bold text have not yet been read by clinic staff. Visits View your upcoming appointments You can view your future appointments in the Upcoming Appointments feature (Visits > Upcoming Appointments). Select a scheduled appointment to see info, such as: The date, time, and location of the visit Any pre-visit instructions from the clinic Document Title: MyChart Version Page 19 of 26 View your After Visit Summary for a past appointments or admission To access a visit summary for a previous appointments in DHA MyChart, go to Visits > Visit Summaries and select the appointment you want to review.

10 This feature might be helpful if you've lost the summary or patient instructions from your last visit, or if you want to review the procedures that were performed. Document Title: MyChart Version Page 20 of 26 Cancel an appointment Depending on the date and time of your next appointment, you might be able to cancel it through DHA MyChart. If it is too close to your appointment date or time, you'll need to call the clinic to cancel your appointment. To cancel an appointment: 1. Go to Visits > Cancel Appts, and select the appointment from the list. Click Continue. 2. Enter cancellation comments. 3. Click Confirm Cancellation. Document Title: MyChart Version Page 21 of 26 My Medical Record View your test results With DHA MyChart, you can view test results as soon as they become available, rather than waiting for a phone call or letter from your physician.
