1 Important Instructions for China visa Application The China visa Application is below on Pages 2 - 5. Please review these Important notes. The Application form must be typed in ALL CAPITAL LETTERS. Handwritten answers, including corrections are not accepted by the Chinese Consulate. On the form it says If some of the items do not apply, please type N/A or NONE . Certain questions cannot be N/A, if the question has a yes or no answer you must state NO or NONE . N/A is only if the question does not apply to you at all. The following questions may not be left blank or answered with N/A: Please provide your Last, Middle and First Name exactly how they appear in your passport. Do not use nicknames or initials unless displayed as so in your passport.

2 If you do not have a Chinese name, please enter NONE . If you were born in China to Chinese parents you must write a Chinese name in Chinese characters (this is one exception, this answer can be handwritten in black ink if you do not have Chinese script on your computer). If your citizenship has not changed since birth, please enter NONE . US residents should enter Drivers License Number or Passport Number. Canada residents please use Drivers License Number or SIN Number. If this field is left blank, you will be required to complete another Application . If you have never been granted a Chinese visa , enter NO . If you have not visited any other countries in the last 12 months, enter NONE. Section 4 must be signed by the applicant, unless a young minor child.

3 Section 5 asks for signature if the Application form is completed by another person on the applicant's behalf; this is for a parent in case of a young child. Otherwise only the applicant's signature is permitted. Please make sure that no other questions left blank on the Application .
