3 Verizon Protect consists of: see above for TMP description, plus Digital Secure $ ; and Call Filter Plus $ (each available separately). Verizon Protect MD (VZP MD) is $50 for 3-10 eligible lines (requires a minimum of 3 smartphones on the account). Verizon Protect MD includes account level Digital Secure ($ ) and Call Filter Plus ($ ), both separately available. Additional Coverage (available for Consumer TMP MD and VZP MD only) is available for up to 7 additional lines for $ Business: TMP for Business is $ for Tier 1 Smartphones and Watches and $ for Tier 2 Smartphones and Watches, Basic Phones and Tablets. 4 Covered devices Your device and standard accessories1 (if included in the loss) are covered if lost, stolen or damaged2 and/or experience a post-warranty defect3. Your replacement could be new, refurbished or remanufactured and may contain original or non-original replacement parts. Same color and features are not guaranteed for your replacement device . Claim limit For all single-line coverage programs (VMP, TEC, WPP, Verizon Protect*, TMP*, TMP for Business, Verizon Protect Business: 3 claims (excluding approved cracked screen repair claims) allowed in any consecutive 12-month period, with a per claim max of $400 or $3,000, depending on device . For the following account-based coverage programs: VMP MD (not available in NY): 3 registered lines share 9 claims (excluding approved cracked screen repair claims) in any consecutive 12-month period with a per claim max of $400 or $3,000 depending on device .)

5 Claims roll off after 12 months without a claim on that registered line and free up a registration. Customers with more than 3 eligible lines (and no more than 10) on their account can enroll in Additional Coverage to register 1 more line and get 3 additional shared claims (excluding approved cracked screen repair claims). TMP MD* and Verizon Protect MD*: 3 registered lines share 9 claims (excluding approved cracked screen repair claims) in any consecutive 12-month period with a per claim max of $400 or $3,000 depending on device . Customers with more than 3 eligible lines (and no more than 10) on their account can enroll in Additional Coverage to register 1 more line and get 3 additional shared claims (excluding approved cracked screen repair claims). TMP MD for Business (non-NY customers): for accounts with 3-10 lines 3 registered lines share 6 claims; for accounts with 11-24 lines, 11 registered lines share 22 claims; for accounts with 25-49 lines, 25 registered lines share 50 claims (excluding approved cracked screen repair claims) in any 12-month period with a per claim max of $400 or $3,000 depending on device .

6 TMP MD for Business (NY customers only): for accounts with 3-10 lines 3 registered lines share 9 claims; for accounts with 11-24 lines, 11 registered lines share 33 claims; for accounts with 25-49 lines, 25 registered lines share 75 claims (excluding approved cracked screen repair claims) in any 12-month period with a per claim max of $400 or $3,000 depending on device . Claims roll off after 12 months without a claim on that registered line and free up a registration. *Available in New York Only Deductibles For all non-New York customers, the deductible for a replacement claim will be $19/$129/$189/$229/$249, depending on device . For a smartphone eligible for cracked screen repair, the deductible is $29 per approved claim (except for TMP MD for Business customers their cracked screen repair deductible is $19 per approved claim).

7 For programs in New York (or for New York customers), the deductible for a replacement claim will be $9/$99/$149/$199/$249, depending on device . New York customers with a smartphone eligible for cracked screen repair have a $29 deductible per approved claim. Cancellation policy device protection automatically renews each month unless canceled and is billed directly to your wireless account. Enrolled customers may cancel anytime and receive a prorated refund of their Monthly fee. device Requirements (if applicable to my transaction) Open Enrollment: I agree that by continuing with this transaction, I am representing that my device is fully functioning, without damage (including cracked screen), and free of defects. 8 Certain BYOD or Verizon devices may not be eligible for coverage. Talk to your sales representative about device eligibility.
