3 COURT has jurisdiction over child custody and parenting timeissues pursuant to 43 551-101 et seq. because the COURT s current childcustody determination was consistent with the provisions of the UCCJEA. B.[Choose one of the following:] _____The parties agree and the COURT so finds that appointment ofa Parenting Coordinator is necessary to assist the parents ini mp l e m e n t i n g t h e t e r ms o f t he e x i s t i n g c h i l d c u s t o dy a n dparenting time order for the specific issues listed in paragraph 5below.

4 _____The COURT finds that the appointment of a ParentingCoordinator is in the best interest of the child/ren and that this is ahigh conflict case because the parties have demonstrated a patternof ongoing litigation; anger and distrust; verbal abuse; physicala g g r e s s i o n o r t h r e a t s o f p h y s i c a l a g g r e s s i o n ; d i f f ic u l t y i nc o mmu n i c a t i n g a b o u t a n d c o o p e r ating in the care of theirchild/ren; or conditions that, in the discretion of the COURT ,warrant the appointment of a Parenting Coordinator.

5 Coordinator., or his/her successor, is appointedas a Parenting Coordinator. The parties shall cooperate with the ParentingCoordinator and follow the terms specified in this - Form approved by term of the Parenting Coordinator s service shall be for a period of_____ years from the date of this order. If the Parenting Coordinator requeststo withdraw, then the Parenting Coordinator shall appoint another ParentingCoordinator who is agreeable to both parties.

6 If the appointment is not made,then the COURT will appoint a new Parenting Coordinator. If the Courtterminates the appointment prior to the expiration date, notice shall be givento the Parenting Coordinator, and a hearing Subject to Parent Authority. The Parenting Coordinator will aid the partiesin: (1)Identify disputed issues;(2)Reducing misunderstandings;(3)Clarifying priorities; (4)Exploring possibilities for compromise; (5)Developing methods of collaboration in parenting; and (6)Complying with the COURT s order of custody, visitation orguardianship.

7 Making Power. In order to carry out the general authoritygranted to the Parenting Coordinator, the following specific issues aresubmitted to the Parenting Coordinator for decision (to be filled in by the Judgeonly): (1)(2)(3)410/02/03 - Form approved by QAP.(4)(5) to Power. The Parenting Coordinator shall not make anymodifications to any order, judgment or decree in this case; however, theParenting Coordinator may allow the parties to make minor, temporarydepartures from the parenting plan if authorized by the COURT to do so.

8 TheParenting Coordinator shall not divest the COURT of exclusive jurisdiction todetermine the fundamental issues of custody, visitation and child support. of Parenting :The Parenting Coordinator shall decide any issue asset forth in paragraph 5 within the scope of his/her authority by anyappropriate dispute-resolution method. During this process, the ParentingCoordinator may coach and educate the parents about ways to bettercommunicate about the child/ren and about child development issues.

9 :(1)The Parenting Coordinator may recommend to the Courtthat the parents participate in any of the following interventions: therapy, recognizing that couples counseling is not appropriate incases involving domestic violence; treatment programs, including batterer s intervention courses; mediation;510/02/03 - Form approved by QAP. custody/parenting time evaluation; programs for high conflict parents, anger management, etc.; and interventions for the minor child/ren including the appointment ofan attorney or therapy.

10 (2)The Parenting Coordinator may recommend changes in theParenting Coordinator s decision-making power to better accommodatethe needs of the parties or their children that may arise during theparenting coordination. (3)Any recommendations shall be submitted to the COURT inwriting with copies to each parent and attorneys involved in the judge shall determine whether a modification is necessary toimplement recommendations of the Parenting.
