NATIONAL EXERCISE REFERRAL SCHEME FALLS PREVENTION INCLUSION / EXCLUSION CRITERIA INCLUSION CRITERIA A result of 1-3 from functional assessment grid will indicate referral to PSI A result of 4-5 from functional assessment grid will indicate referral to chair based PSI Mobile (with walking aid if needed, but no human assistance) Able to get out of low chair/ up from toilet without assistance. EXCLUSION CRITERIA Unstable angina or uncontrolled heart disease Tachycardia or uncontrolled arrhythmias >100bpm Uncontrolled Hypertension >180/100 mmHg (in either) Severe Parkinson s Disease Severe breathlessness or dizziness Unmanaged pain Acute systemic illness ( cancers) Functional limiting diseases, such as severe stroke.
