2 In the case of Denver Health, their standard flu vaccine confirmation email is sufficient. A picture of a badge sticker is not acceptable as documentation. To receive a flu vaccination at the hospital where you are rotating at no cost to you: Present your hospital specific ID badge to obtain your flu vaccination at that hospital. Available information regarding hospitals that offer flu vaccines to RESIDENTS and fellows rotating in their facility is below. Check in-hospital communications for additional information, updates and changes to flu vaccination clinic schedules. To receive a flu vaccination through your health insurance coverage at no cost to you: RESIDENTS , fellows and their dependents enrolled in the CU GME Health Benefits Plan may obtain a flu vaccination through in-network providers for the CU GME Health Benefits Plan.

3 Visit your in-network PCP or an in-network pharmacy that offers flu VACCINATIONS . Be sure to have your CU GME Health Benefits Plan ID card with you and remember to obtain documentation of the vaccination at time of service for submission to MedHub. 4 -You will receive a sticker for your UCH ID badge. -When you get your flu vaccination at a UCH flu vaccination clinic, obtain a copy of your consent form at time of service. 5 -Pre-registration is mandatory PRIOR to attending a vaccination clinic. DHHA will have a computer at COSH for registration if you decide to get your vaccination at COSH. (It is MUCH easier to take care of this in advance.) -After you receive the flu vaccination, DHHA will send you an email that serves as your documentation. This is the documentation you are required to upload into MedHub. -REVIEW and UPDATE your email address when you pre-register. Your documentation is sent to your DHHA email address unless you provide a different email address. CHILDREN S HOSPITAL COLORADO (CHCO): -Refer to in-hospital communications for flu vaccination clinic changes, updates and additional information. -When you get your flu vaccination at CHCO: -Present your Health Plan ID card (CU GME Health Benefits Plan or ID from alternate ins).
