2 1. Makes patient care referrals to outside agencies according to Northland Health Centers policies, procedures and responsibilities. 2. Documents all referrals and forwards Referral documents to appropriate personnel / department. 3. Follows up the referrals in a timely manner to assure that appointments are made for appropriate care or testing. 4. Answers patient inquiries and takes messages for follow up. 5. Assists providers in completing all medical documentation needed to provide Referral services following the specific procedure provided by Nurse Supervisor as to what needs to be sent to specific entities.

3 6. Participates in staff meetings and other meetings as instructed. 7. Maintains confidentiality within the workplace and within the community. 8. Handles protected health information in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). 9. Adheres to the Mission and Values of NHC. 10. Performs other duties as required and assigned. Position Requirements: 1. Excellent communication skills 2. Attention to detail 3. Computer knowledge 4. Ability to organize and prioritize tasks 5. 4 Current Immunization Records 8. Current PPD. Minimum Requirements: 1. Ability to understand that safety is a condition of employment. Unsafe acts or conditions will be reported to the supervisor. 2. Must be able to read, write and speak English. Education and/or Experience: High School Diploma Previous Medical Referral Experience and/or CNA certification PREFERRED. License and/or Certification: Valid North Dakota Driver's License . Special Requirements: Must be willing to travel between NHC health centers. Must be willing to work evening hours and weekends if necessary. While performing the duties of this position, the employee is exposed to weather conditions prevalent at the time.

5 The employee may experience exposure to dust, pollens, pollutants, fumes, and communicable diseases as related to the health care environment.
