1 LOCAL HEALTH & CARE RECORD EXEMPLARS CALL East London Leading for Pan London Response to Call 16th January 2018 Introduction NHS England is about to launch a call for proposals for up to five LOCAL HEALTH and Care RECORD Exemplar (LHCRE) programmes across populations of 3-5m that can raise the bar in how the NHS, and its partners, share data to help deliver better care for our citizens. Each exemplar will be granted available from 18/19 to 19/20 for each locality matched with LOCAL investment and resource to implement and roll out their exemplar programme. Up to 5 of these will be awarded nationally.

2 LHCRE Expectations The expectations of the selected EXEMPLARS will be that by March 2019 they will be able to demonstrate how: all HEALTH and care professionals involved in a person s care have access in real-time to a comprehensive care RECORD comprising the pertinent individual level information they need to inform and enhance their care decisions, when and where they need it; combining and analysing the underlying data in the RECORD enables delivery of more precise and optimally effective interventions the development of population HEALTH management addresses upstream exposures (diet, air quality, physical activity, smoking, alcohol, sexual HEALTH )

3 And interventions including patient behavioural change, screening and anticipatory care they empower citizens and carers to manage their own care by giving them access to their own healthcare records, they can work alongside research, academic institutions and public HEALTH in providing a rich information base of de-personalised and anonymous information to support research into conditions, the development of new treatments and improved and innovative pathways for care and prevention. Pan London LHCRE Bid NEL and SEL STPs are in agreement to develop a response and be the locality to represent Pan London.

Assessment, selection and identification of successful sites. The announcement of successful sites (subject to formalised MOU) at the end March 2018 . April 2018 end May 2018 . Further detail with sites, leading to a memorandum of understanding with each locality
