5 ACCOMMODATION - Chapter 1 Mildmay House is a 48 bed supported accommodation project. The accommodation comprises of 37 en-suite bedrooms with shared kitchens and lounges and 11 fully self-contained flats used in preparation for move on. Mildmay House is fully furnished and all new clients are provided with essential move in items such as bedding and toiletries. Mildmay House has a large internet style caf area in the centre of the building on the ground floor providing a relaxed space for clients and 24 hour internet accessible computers, additionally there is a training kitchen, pool table and television. Mildmay House also has a small gym located on the lower ground floor accessible following an induction, music studio and our treatment room is utilised by outside agencies to deliver in-house essential healthcare. Mildmay House also has a lift and stair lift providing an environment that is accessible and inclusive.

6 Mildmay House is self-catering with exception to our breakfast club and weekly Sunday Dinner. ACTIVITIES ON OFFER- Chapter 1 runs a host of in-house informal and structured activities and include; Fishing Football Bingo Pool Competitions Get Creative sessions Literacy sessions Doorstep Sports Club (pop up sports) Table Tennis Cinema Breakfast club three days a week Ready Steady Cook Walking Trips Camping Trips Film and games nights Tenancy Training Maths sessions Volunteering Playstation Shopping Managing on a low income There are also one to one sessions which include language, CV and job searches. Chapter 1 also runs a social enterprise called Bloomin Baskets in which clients make hanging baskets and sell them at local markets. All the proceeds go back into Chapter 1 to fund further activities. Our activities programme is driven by client need and evolves as new needs or interests are identified.

7 Chapter 1 prides itself on providing a menu of activities for clients and acknowledges that meaningful activity is key to successfully moving on from sometimes negative and destructive lifestyle choices. Our activities staff work around you and ensure that they will make time to support you refresh/ develop essential skills for the future. In addition to this Chapter 1 is proud to inform you that from the 9th June 2016 we are able offer onsite prescribing SERVICES for dependant drug users through an exciting partnership with NHS. 8 (soap powder 20 pence per wash) Treatment Room for access to health care Training kitchen CCTV recording 24 hours En-suite rooms Page 6 of 55 SERVICE DESCRIPTION - Once you have been accepted into Chapter 1 Mildmay House you will be supported by an Allocation Worker whose main role is to help you settle into Mildmay and start working with you to explore areas of support you require. They will work with you for the first two weeks and support you with prioritising essential things such as setting up benefits and getting you registered with a GP. After the first two weeks you will be passed over to your support worker after successful completion of a needs assessment, your allocated worker will support you to achieve all the goals that have been identified. Your support package will be personally tailored for you and is reviewed on a quarterly basis. Engagement with your allocated worker is a condition of your Licence Agreement and the expectation is you will meet with your support worker on a regular basis.

9 Once it is agreed you are ready to move on and you have completed our in-house six week tenancy training sessions you will be eligible to move to one of our self-contained flats whilst waiting for a successful offer of permanent accommodation through agreed housing routes such as PropertyPool. Support is also available through our own resettlement worker to ensure a successful transition to independent living. All clients leaving Mildmay House in a planned way are eligible to continue accessing the facilities as part of our commitment to ensuring successful move on. SERVICE CRITERIA- Chapter 1 Mildmay House works with individuals is open to men and women aged 16 plus who do not have access to settled accommodation or at risk of homeless. Dogs will be only considered at point of offer of accommodation, subject to information recorded on MAINSTAY , completion of a risk assessment and a commitment to working with the PDSA.

ACCOMMODATION DESCRIPTION- There are 8 units of accommodation across in three locations in L3, L6 and L15. 6 units of the accommodation within the complex needs with personal care service are mobility accessible and all of the units are en suite.
