1 How to Check Your Medical Readiness Status Periodic Health Assessment (PHA) Labs (Current HIV Blood Draw, DNA) ImmunizationsIf you are REDfor any of the following reasons:HIV Blood Draw: Lab is a walk-in service at LAHC, 0730-1530 DNA: Lab is a walk-in service at LAHC, 0730-1530 Immunization(s) : Immunizations is a walk-in service 0730-1530 (All students arriving from West Point will need to go to the Immunization Clinic, Redor Green)Vision and/or Hearing : Flight and WOCC Students will be getting a Flight Physical and PHA soon. No need to go to Eye/Ear Clinic because this will be covered as part of the Flight Physical/PHA*All other personnel can walk-in to Eye/Ear Clinic to Flight Students/WOCC students All Flight students will receive a new PHA as part of the upcoming Flight PhysicalComplete the Member portion of the PHA online priorto the Flight Physical.

3 CLICK HERE ONCE THE MHA HOMEPAGE IS ACCESSED, CLICK ON THE PHA TAB AFTER CLICKING THE PHA TAB, CLICK THE START NEW SURVEY BUTTON TO BEGIN A NEW FORM. COMPLETE MEMBER PORTION, CONFIRM DATA, AND SAVE FORM. CONFIRM THAT THE PHA STATUS READS MEMBER COMPLETE IF IT DOES NOT SAY MEMBER COMPLETE , CLICK ON VIEW , UPDATE FORM, AND SAVE.
