Contacts can be found here: Environment Inspectorate objective of this form is to assist Small operators in the preparation of a Mining Proposal and Mine Closure Plan. This form is to be used for Small -scale developmental Mining and is not intended to replace the Programme of Work form which can still be used for exploration and prospecting activities. To assist with completing this form, explanations have been included, with further information available in the Guidelines for Mining Proposals in Western Australia (2006) and Guidelines for Preparing Mine Closure Plans (2015).

A Mining Proposal and Mine Closure Plan for Small Mining operations is required to meet the following criteria:1. low risk, Small -scale activities, including scrape and detecting, Small -scale test pits, Small mobile dry/wet plants, Small -scale vat/heap leach operations, Small shafts or winzing, Small precious stones Mining , Small -scale gypsum Mining , Small -scale basic raw material extraction and Small -scale river-sand Mining . The assessment of risk is based on the type, scale, location or scope of activities to be operations which are free of high risk issues that may require ongoing management post Closure , for example fibrous minerals, radioactive materials, elevated heavy metals, material capable of generating acid and/or metalliferous drainage, erodible material that is capable of compromising the stability and or safety of the waste dump or overburden stockpile, permanent surface water operations which do not include permanent excavations below the water table.
3 4. operations with less than 4,000 litres of fuel on operations which are not located within reserved land ( conservation, water or town site reserve, with the exception of unoccupied town sites) or Public Drinking Water Source operations which do not require referral to the Environmental Protection Authority under Part IV of the Environmental Protection Act 1986. This refers to significant proposals likely, if implemented, to have a significant effect on the environment. 7 2. SITE LAYOUT PLAN A site plan is attached which includes: All proposed and existing site infrastructure and activities ( outline of excavations/workings, camp site, tracks, etc.). Tenement boundaries and labels. Access tracks and roads, including haulage routes and road/rail transport routes. Aboriginal Heritage sites. Other land tenure including freehold (private) land lots, reserves, proposed reserve boundaries, and pastoral lease boundaries. A north indication. A scale bar. A key or labelling of all infrastructure and activities. 53. EXISTING Describe the existing area ( historic mines, waste dumps, old plant sites, general rubbish/scrap, large previously cleared areas) Describe Existing Vegetation and Landform ( Mulga shrublands, Eucalypt/saltbush woodland, Spinifex grassland, Flat alluvial plain, greenstone, range, creek/drainage lines, hillsides etc.)
8 A flora and fauna desktop enquiry/search is required to determine if there are any known species of conservation significance ( threatened, priority, declared rare flora and fauna species) within the area. At a minimum, a search of the project tenements is to be conducted on the Department of Biodiversity, Conservation and Attractions (DBCA) NatureMap website (link: ). The search must be undertaken within 3 months of submission of the Mining Proposal . Please contact a DBCA or DMIRS Environmental Officer if you require assistance using the search identifies flora or fauna of conservation significance, then further work, including on-ground surveys, may be required. This should be discussed with the relevant DMIRS Environmental Officer. DMIRS recommends the applicant includes photos of the proposed disturbance area to assist the assessment process.
