Instructions to Bidders iv MINISTRY OF HEALTH Invitation for Bids MoH/ORD//14 - Tender for the Supply, Delivery of Medical Equipment for 125 HEALTH Post in Central,Copperbelt,Eastern,Luapula,Lusak a,Northern, Northwestern Southern and Western Provinces The MINISTRY of HEALTH has released funds from SIDA and it intends to apply part of the proceeds to payments under the contract for the Tender for the Supply and Delivery of Medical tender will be processed under International Competitive Bidding (ICB) procedure.

The MINISTRY of HEALTH now invites sealed bids from eligible and interested local bidders for the Supply and Delivery of Medical Equipment for 125 HEALTH Post in Central, Copperbelt,Eastern,Luapula,Lusaka,Northe rn, Northwestern, Southern and Western Provinces as follows: Lot 1: Medical Equipment for Maternity wards for 125 HPs - 1,000,000 Lot 2: Procurement and Installation of Radios for 125 HPs - 250,000 Lot 3: Procurement of Cold chain equipment for 125 HPs - 350,000 2. The MINISTRY will evaluate the bids on Lot by Lot basis. Bidders not quoting for all required quantities per Lot will be considered non-responsive. 3. A complete set of bidding documents may be purchased from the MINISTRY of HEALTH , upon payment of a non-refundable fee of K or its equivalent in any freely convertible currency at the prevailing exchange rate, in cash or Bank certified cheque. 4. Bids should be sent clearly marked Tender for the Supply, Delivery, Installation and Commissioning of Medical Equipment for 125 HEALTH Post in Central, Copperbelt,Eastern,Luapula,Lusaka,Northe rn, Northwestern, Southern and Western Provinces and addressed to the Head Procurement and Supplies Unit, MINISTRY of HEALTH , Ndeke House, Haile Salessie Avenue, Longacres Box 30205, Lusaka, Zambia.

Scope of Bid The Procuring Entity indicated in the Bidding Data Sheet (BDS), issues these Bidding Documents for the supply of Goods and Related Services incidental thereto as specified in Section VI, Schedule of Requirements. The name and identification number of this Open International Bidding (OIB) procurement are specified in the BDS. The name, identification, and number of lots of are provided in the BDS.

10 Throughout these Bidding Documents: (a) the term in writing means communicated in written form ( by mail, e-mail, fax, telex) with proof of receipt; (b) if the context so requires, singular means plural and vice versa; (c) day means calendar day; (d) the term Contract Manager refers to the officer, body or institution appointed under Section 57 of the Public Procurement Act of 2008; (e) Government refers to the Government of the Republic of Zambia, any Procuring Entity or the relevant approvals authority as defined in the Public Procurement Act of 2008; and (f) ZPPA refers to the Zambia Public Procurement Authority. 2. Source of Funds The Procuring Entity specified in the BDS has applied for or received financing (hereinafter called funds ) toward the cost of the project or programme named in the BDS. The Procuring Entity intends to apply a portion of the funds to eligible payments under the contract for which these Bidding Documents are issued.
