If test requested on MADL requisition is not listed on this page, order AI MISC. Use a single AI MISC when there is more than one AI MISC test. If AI MISC is used, type the test name into the AI Test Name field. If there are multiple test names or if test name doesn t fit in the AI Test Name field, type See Comment in the field and append an Order Comment, typing the test names exactly as written on the requisitions.

2 If test is requested on a Mitogen Immune Biomarker Test requisition (Cytokine), order a MISCREFBL and follow collection instructions on the requisition or APL Test Directory page for Cytokine.
