2 Total 55 Nos. of Safety trainings conducted at site and achieved 1110 man months during the month of January 15. Total Plants (like Batching Plants, Crusher Plants, and Hot Mix Plants etc.) have been inspected during the month of January 15 by HSE team and, mechanical team. Total 4283 man-month achieved from tool box talks such as Incident alerts, Hazard & Fire safety to site etc. conducted at site. 45 Persons safety Induction conducted during the month of January 15. Total 2 nos Housekeeping Inspection conducted at site all during January 15. 09 Nos. Work Permit has been issued for excavation work during January 15. Issued mandatory PPEs to the workers and ensure its proper use at site and PPEs stock list has been attached with the Monthly Report as Appendix 1.

4 Zero No of Personal Damage Accident until completion at the Construction Site. PD-1 Fatality - 0 PD-1 LTI - 0 No-Major Accidents 2 Reducing personal damage Class 2 and Class 3 incident systematically. PD Class-2:- Damage which temporarily alters a person s future broken arm or muscle strain etc. PD Class-3:- Damage which inconveniences a person first aid treatment for a cut hand, dust in eyes etc. Near Miss / Dangerous Occurrence Achieve million safe man-hours worked since last LTI on 1st July 12 PD -2 (LTI) - 0 PD -3 - 3 LTIFR - 0 The project has achieved million safe man-hours worked since last LTI on 1st July 12 3 Increase a level of safety and health awareness among employees and sub-contractors.

5 Conducted Tool Box Talk before starting of work. Site Inspection with Checklist. Total man-month for Tool Box Talk achieved 4283 Nos. Total 55 nos. of training programmes Conduct at the site. Total man-month for training achieved 1110 Nos. Conducted Monthly HSE Committee meeting at site for review of HSE compliances and performance. Tool Box Talk conducted before starting of work by HSE Team. 26th HSE Committee meeting held on dated 29th January 15. 4 To maximise sub-contractor and supplier involvement in improving Safety and health performance. Involve the sub-contractors and their employees in all HSE training programmes. Being Done 5 PUC, Driver licence & other documents PUC, Driver licence & other documents should be available in construction vehicle.

6 PUC & fitness certificate of all required construction vehicle has been update as on date. 6 EMP Documentation Environmental Performance Assessment checklist had not been maintained. It should be maintain on Monthly basis. Environmental inspection has been done and other documents shall be Monthly HSE Report January 15 Page 5 of 8 Jetpur-Somnath Road Project maintained as per requirement. 7 HSE Training HSE Training Should be conducted as per training plan. Training conducted as per scheduled of Monthly plan. Total 1110 workers participated during this safety training 8 Excavated Material stacking at working site. Excavated Material should be stack up to 2m height at working site. Excavated material removed from the edge of excavated pits Objectives Measures Progress Environmental: Implementation of EMP Plan for Jetpur-Somnath Project Environmental mitigation measures has been implemented at site Complied Qtr.

8 Safety training conducted at site Over 1110 Nos. Worker participated in this training including Site engineers of EPC Contractor and sub-contractors have successfully attended the above training programme. The site training is a step towards our awareness programmes conducted on Monthly basis on HSE aspects among all workman and supervisory staff to create awareness among project staff for safe, secure and healthy environment at work site. Every day tool box talks conducted at site, before starting of work at each working location. Over 4,283 Persons including Engineers and Supervisors participated. Key Action for January 15: Status of Key Actions from previous month: Health & Safety: Conducted 26th Road Safety Week Campaign during 11th to 17th of january 2015 Conducted Maintenance of Diversion boards and supporting boards during the month Installation of Metal Beam Crash Barrier is started and work under progress.

9 Road Markings started and work under progress in 4 lane completed stretch and junctions Installation of Permanent Road Signs started and work under progress in Project. Installation of Road Studs (cat eyes) at median openings, Curves, and Built up areas Safety Induction training programs for third party Agencies of Road furniture, lane Marking, Sign board contractors Specific HSE Training programs conducted during month covering welding, cutting, rigging, material handling, hot work, asphalt works etc Housekeeping of work site during the month PPE s Inspection and replacement during the month Environment: Ensured water sprinkling system (water tankers) at construction site for dust pollution control.

10 Environmental Monitoring along the Project road and plant site has been conducted on 17th Dec 2014. Green belt developed near all Plants (like Batching Plants, Crusher Plants, Hot Mix Plants and Camp office etc. Installed water sprinkling system in all the crusher plants Rehabilitation of Borrow area and Quarry sites to be initiated after completion of project.
