2 How to use the checklist: 1. Complete visa application Form 1419. 2. Use this checklist to make sure that you have included all relevant forms and supporting documentation 3. Sign and date your checklist and visa application form 4. Submit your completed application (including documents and checklist) at the Australian Visa Application Centre (AVAC). Delivery and courier charges You are responsible for all costs of delivering information to the Australian Embassy by mail or courier, including any additional information that may be requested by the Australian Embassy. If you give additional information to the AVAC, you may be charged a courier fee for delivery to the Australian Embassy. Refunds The Visa Application Charge will not be refunded if a decision is made to refuse to grant the visa because the applicant did not satisfy the criteria for grant of the visa. Original documents Please do not include original documentation or original photographs with your application.

3 Original documents may not be returned. Original identity documents should be presented to AVAC at the time of application, and certified copies should be submitted with your application. If you have not provided your own certified copies, AVAC is able to certify documents for a fee. Documents not in English must be accompanied by accredited English translations. FORMS . Payment of the Australian Government Visa Application Charge Advice on how to pay this charge can be found at or if the Visa Application Charge has been paid at an office of the Department of Immigration and Border Protection in Australia, a copy of the receipt must be submitted IDENTITY DOCUMENTS . Passport Certified copy of each page of the applicants' valid passport Photograph One recent passport sized photograph (45mm x 35mm). 5 APPLICANTS UNDER THE AGE OF 18 . Form 1229. Form must be completed and signed by both parents as per the child's birth certificate If the child does not have contact with both parents, additional documents must be provided to demonstrate this. For example: o Deceased parent: Death Certificate o Sole custody: Custody Court Order Parents' identification documents A certified identity document (passport or national identification card) must be provided by each parent signatory to form 1229. PERSONAL (BUSINESS, EMPLOYMENT, STUDY) . If you are employed Letter from your employer on the companies letter head stating your position, duration of service, salary and period of approved leave If you are self employed Certified copies of company documents such as business registration license, business patent and bank statements If you are unemployed or undertaking home duties Signed letter from your partner, relative or other supporting person, declaring their support of your application If you are a student Evidence of enrolment A letter from the institution showing the duration / dates of approved leaves Other documents Evidence of immediate family members in your home country Evidence of property or other significant assets owned in your home country Evidence of any recent international travel (within the past 5 years) passport stamps, visa labels PURPOSE OF TRAVEL.
