Applicant Name: Passport Number: The Visitor Visa ( TOURIST STREAM ) is for applicants seeking temporary entry to Australia for TOURIST purposes or to visit family and friends. Applicants who seek to partake in business activities in Australia should apply under the Business STREAM of the Visitor Visa. Each application is assessed based on the applicant's individual circumstances as evidenced with supporting documentation. The information provided below is a guide only. The visa application checklist helps you check that you have included all of the necessary documentation that is needed by the department to process your visa application.

2 How to use the checklist: 1. Complete visa application Form 1419. 2. Use this checklist to make sure that you have included all relevant forms and supporting documentation 3. Sign and date your checklist and visa application form 4. Submit your completed application (including documents and checklist) at the Australian Visa Application Centre (AVAC). Delivery and courier charges You are responsible for all costs of delivering information to the Australian Embassy by mail or courier, including any additional information that may be requested by the Australian Embassy. If you give additional information to the AVAC, you may be charged a courier fee for delivery to the Australian Embassy. Refunds The Visa Application Charge will not be refunded if a decision is made to refuse to grant the visa because the applicant did not satisfy the criteria for grant of the visa. Original documents Please do not include original documentation or original photographs with your application.

3 Original documents may not be returned. Original identity documents should be presented to AVAC at the time of application, and certified copies should be submitted with your application. If you have not provided your own certified copies, AVAC is able to certify documents for a fee. Documents not in English must be accompanied by accredited English translations. FORMS . Please note that a separate application form must be completed for each person travelling Form 54 Family composition The information required is about the composition of your family and is not limited to the people you currently reside with If you are unable to answer parts of this form, please write unknown' in the corresponding field Form must be completed and signed Form 956 / 956A Advice by a migration agent/exempt person of providing immigration assistance This form is optional. Payment of the Australian Government Visa Application Charge Advice on how to pay this charge can be found at or if the Visa Application Charge has been paid at an office of the Department of Immigration and Border Protection in Australia, a copy of the receipt must be submitted IDENTITY DOCUMENTS . Passport Certified copy of each page of the applicants' valid passport Photograph One recent passport sized photograph (45mm x 35mm). The photo should be no more than 6 months old for each applicant The photo must capture the applicant's head and shoulders only, against a plain white or blue background Print the name of the person on the back of each photograph Optional documents National identification card Certified copy Household member list Certified copy Birth Certificate Certified copy FUNDS . Evidence that you have access to sufficient funds to finance your proposed visit This can include personal bank statements (including transaction history), payslips or taxation records If the applicant is not self funded, the following documents from the financial supporter: o Evidence of funds o A letter declaring financial support o Identity document ( passport, driver's license, identity card).

5 APPLICANTS UNDER THE AGE OF 18 . Form 1229. Form must be completed and signed by both parents as per the child's birth certificate If the child does not have contact with both parents, additional documents must be provided to demonstrate this. For example: o Deceased parent: Death Certificate o Sole custody: Custody Court Order Parents' identification documents A certified identity document (passport or national identification card) must be provided by each parent signatory to form 1229. PERSONAL (BUSINESS, EMPLOYMENT, STUDY) . If you are employed Letter from your employer on the companies letter head stating your position, duration of service, salary and period of approved leave If you are self employed Certified copies of company documents such as business registration license, business patent and bank statements If you are unemployed or undertaking home duties Signed letter from your partner, relative or other supporting person, declaring their support of your application If you are a student Evidence of enrolment A letter from the institution showing the duration / dates of approved leaves Other documents Evidence of immediate family members in your home country Evidence of property or other significant assets owned in your home country Evidence of any recent international travel (within the past 5 years) passport stamps, visa labels PURPOSE OF TRAVEL.

6 If you are travelling for TOURIST purposes only Information regarding your proposed travel plans If you are travelling to visit an Australian friend, partner or family member Evidence of your relationship to the Australian birth certificates, family books, photographs Letter to declare the Australian's support of your application, your plans in Australia and the nature of your relationship Identity document from the Australian supporter HEALTH REQUIREMENT .
