Weask you to pick a second choice in case your first choice is not the names and the Provider numbers of the primary care providers that you picked on the enclosed PrimaryCare Provider Selection Form. Call our Members Services Department to make sure the doctor or medical group the Primary Care Selection Form to PHC using the enclosed postage paid return can access some Medi-Cal benefits without an authorization from your PCP. A specialist is a doctor who has extra education in one area of medicine. Your PCP will work with you to choose a specialist. For some types of care, your PCP or specialist will need to ask for approval before you get the care. This is called prior authorization or pre-approval. It means that PHC agrees that the care is medically necessary. If you have any questions concerning handicapped accessibility, please call our Member Services Department at (800)863-4155.
