If you need specialty care, your PCP will coordinate with our case management department for these services. Specialty services require pre-authorization which must come from your PCP. CCA 11 /2017 2 Emergency Room follow-up Care Dear Subscriber, community Care Associates, (CCA) encourages all subscribers who were treated in an Emergency room and need a referral to a specialist for follow-up care, must contact community Care Associates. An authorization is required by Case Management to see any specialist out-side of your Primary Care Physicians (PCP) office. CCA will find a contracted specialist to mini-mize patient liability. CCA cares about your health and will work with you to ensure that you receive the proper care in a timely manner. Follow-Up care is important to ensure a healthy recovery. With proper documentation (ER discharge documents) CCA can issue a referral within 24-48 hours of the request.
