1 10-Sep-2021 COVID-19 MANDATORY Immunization Obtaining Immunization Records To obtain your COVID-19 Immunization records from myHealth Records or MyAlberta Digital ID follow the steps below: STEP 1: Create a MyAlberta Digital ID (It may take up to 10 days to activate your MyHealth Account). STEP 2: Log in to your MyAlberta Digital ID and select My Personal Records STEP 3: Select Immunizations STEP 4: Select Print Immunizations Obtaining COVID-19 Immunization Records 2 10-Sep-2021 STEP 5: Click on All > under Date Range STEP 6: A pop-up window will appear on right side.
2 Click on Custom and select the date range for your first and second dose dates of your COVID-19 Immunization . Click Save. Obtaining COVID-19 Immunization Records 3 10-Sep-2021 STEP 7: Click on All > under Type of Data STEP 8: A pop-up window will appear on right side. Click on Select/Unselect All to remove the checkmarks. Obtaining COVID-19 Immunization Records 4 10-Sep-2021 STEP 9: Select Immunizations from the list and click Save. STEP 11: Click on Export. Your personal health report will appear on the next tab. Obtaining COVID-19 Immunization Records 5 10-Sep-2021 STEP 12: This report will be at least 3 pages long.
3 Save As or Print the entire Record electronically and send it via email.
