5 CCAC Coordinator completes Evaluator Questionnaire and determines a client is incapable of making LTC decisions. All responses to questions must be clearly documented to contribute to determination or incapacity to make LTC decisions. Evaluator Questionnaires without this information will be considered incomplete and will delay processing to acquire completeness Clients MUST be informed of their finding of incapacity and provided with the Rights Information Sheet. 2. CCAC Coordinator determines that there are no other Substitute Decision Makers who could make LTC decisions on the client s behalf.

6 Any Power of Attorney documentation must be explored and exhausted All possible family or other Substitute Decision Maker s must be exhausted and determination that there is no one who can make decisions on the client s behalf 3. Complete the CCAC PGT Confirmation Form Complete the full application and all fields including current location of the client at the time of the application Provide any names of contacts such as friends even if not considered being an SDM PGT may be able to obtain information about the client s wishes, values and belief to assist with decision making Clear Contact information of the CCAC Coordinator is required for communication and follow up The CCAC Coordinator who is familiar with the client should be the Contact person for follow up 4.

7 Gather all necessary documents for faxing to the PGT Office . (PGT PACKAGE) CCAC PGT Confirmation Form Completed RAI Assessment Evaluator Questionnaire Blank Choice Sheet Blank Consent Form List of possible LTCHs for consideration that may meet the client s care needs NOTE: Provide any known financial information to inform accommodation in LTC April 4, 2011 , JG&JS 3 Office of the Public Guardian & Trustee Steps Key Reminders 5. Fax the package of information to the appropriate Treatment Decision Unit Office . See above fax information and fax to the appropriate Office .

8 6. TDC reviews application and confirms receipt and follow up. The TDC will Contact the Coordinator to confirm receipt and discuss any gaps or issues Confirmation of when the TDC will meet with the client to allow for a decision related to LTC 7. TDC visits the client and makes LTC decisions On site visit is made with the client and LTC decisions are made supported by documentation Completed choice sheet and consent will be provided by PGT Coordinator may be involved during or after the meeting with the client. 8. Coordinator forwards the application & Coordinator forwards the application as per the local process to the LTCHs selected 9.

9 Ongoing monitoring and communication by the Coordinator to the PGT. 10 The PGT would then become the Guardian of property. For all other individuals, if it is suspected that the individual may be incapable of managing their finances, a capacity assessment can be requested. If the individual is found to be incapable by a capacity assessor, the PGT will become the Guardian of property.
