Items must be purchased from authorized US-based Tele Vue Optics, Inc. dealers between June 01, 2018 and July 31, 2018. The original box label is your proof-of-purchase. You must send in the original printed label off each item. For labels on Nagler Zoom eyepieces, cut out the metallic letters Nagler Zoom only. Only one form is required to redeem all rebates . This complete form, original US-based dealer sales receipts, and original box labels must postmarked by August 31, 2018.
