1 Reporting My Work and Training Income: Filling Out the Employment/Training Income Report FormYou need to tell your caseworker how much money you and the adults in your family make. 2 We want to make sure that the money you receive from ODSP each month is the right amount. You can help us to do that by filling out this form and getting it in to us on time. When do I need to fill out the Employment/Training Income Report form?If you or another adult in your family are working, or are paid for training, you need to fill out the Employment/Training Income Report form every month. Your completed form tells ODSP how much money you and the adults in your family were paid between the first day of the month and the last day of the My Work and Training Income: Filling Out the Employment/Training Income Report FormOntario Disability Support Program (ODSP)If I didn t earn anything this month, do I still need to fill out the Employment/Training Income Report form?

3 Yes. If you usually report how much you make, you still need to fill out the Employment/Training Income Report form even if you didn t earn anything this you or another adult in your family did not earn money this month, put $0 under the gross earnings section when you fill out the form. NOTE: The money you re paid by your job or training Program in one month will be used to calculate the amount of money you receive from ODSP in the following month. That means that you may receive less money from ODSP the next month. That s why it is really important to save some of the money you earn especially if you think your job or training Program is going to end I need to report my net earnings or gross earnings?

4 You and your adult family members need to report your gross earnings. Gross earnings may include: all of your wages, including any commissions any tips or gratuities you get as part of your job payment for training vacation pay, and calculates your net earnings based on your gross earnings and your family circumstances. We use your net earnings to calculate your ODSP income My Work and Training Income: Filling Out the Employment/Training Income Report Form Gross earnings refer to the money you get before mandatory deductions are made. Net earnings refer to the money you get after mandatory deductions are made. That is, the money you take home. Mandatory deductions are things like income tax, Canada Pension Plan, Employment Insurance contributions and union else do I need to report?

5 In addition to gross earnings, you and adult family members must also use the form to tell us about: union dues any contributions to a pension plan (not including CPP contributions) child care expenses, and Disability -related work will use this information to calculate the earnings that are deducted from your ODSP will my work pay affect my ODSP income Support ?In general, only 50 per cent of your net earnings,(or business profits, if you are self-employed), are deducted from your income Support payment. If you re attending a post-secondary school full-time, this does not apply. We won t deduct your earnings from your ODSP income to your caseworker to find how your earnings or business profits will specifically affect your ODSP income My Work and Training Income: Filling Out the Employment/Training Income Report FormWhat do I do after I fill out the form?

6 After you or an adult family member fill out the form: sign and date it attach copies of your pay stubs, or a letter from your employer or training Program , that shows your gross earnings for the month attach copies of receipts for child care expenses or Disability -related work expenses for the month (if applicable), and mail, fax, or drop off everything to your local ODSP office no later than the 7th day of the following month. For example, you should report your September earnings no later than October 7th. What if I forget to send the form in on time?If we don t receive your form by the 7th day of the following month, we won t be ableto calculate how much money you should receive at the end of the month.

7 That means we may have to temporarily stop sending you your ODSP payments until we receive your completed Employment/Training Income Report the ODSP office receives your form, we can start sending you your payments again, as long as you are still eligible to receive if I stop working or attending a training Program ?If you or an adult family member stop working or attending a training Program , you need to: tell your caseworker right away fill out the Employment/Training Income Report form that month, and include: when your job or training Program ended, and the name of your employer or training Program send the Employment/Training Income Report form to your caseworker may ask you to provide verification that your job or training Program has ended.
