In the Provider Search, select " Sparrow Provider Network."Welcome to Sparrow Physicians Health Network (SPHN) A Primary Care Physician (PCP) includes those physicians in the specialties of Family Practice, Internal Medicine, and Pediatrics, as well as General Practice. If you are a new patient to the PCP s practice that you have selected, it is very important that you contact the office and schedule a new patient visit to establish yourself in their practice. Please note that establishing yourself as a patient of a PCP practice is necessary while you are healthy.

2 The physician s practice will not see you on an urgent basis, or make a referral, unless you have already had a visit to the office. The staff at SPHN would be happy to assist you in finding your new PCP. Physician practices are very busy, so please plan ahead to have your new patient visit scheduled as soon as possible. (Please note that benefit coverage for services received from any provider, including new patient visits, are subject to the terms of your benefit plan.
