2 Key responsibilities , running of activities, setting up and cleaning up after residents in a Rural Northwest Health vehicle to community one on one conversations and activities and support residents to be comfortable at Leisure and Lifestyle programs and residents person centred care plans to ensure thatthe services provided reflects residents requests to support their physical, psychological,social, spiritual and emotional needs, focusing on their capabilities to support them tocontinue to contribute to the that residents, their families and volunteers feel further key responsibilities of this Position refer to allocated duty list and procedures fordaily Position accountabilities Accreditation and governance practice within framework established by legislation, national and state , comply with and adhere to Rural Northwest Health guidelines, principles, policiesand.

6 Detection and information technology as appropriate in the performance of the selection criteria Qualifications and relative experience Certificate IV in Leisure and Lifestyle or willing to undertake same. Experience providing services to people living with dementia. Experience working with and supporting volunteers. Contemporary generalist aged care knowledge and Demonstrated experience delivering a range of Leisure and Lifestyle activities. Ability to adapt and respond to residents needs.

7 Demonstrated competence in delivering services that ensures residents are safe. Good written and verbal communication skills including computer skills. Ability to complete assessments, care plans and reviews within designated time frames. Ability to work as part of an inter-disciplinary team. Commitment to and willingness to participate in continuing training and education related toarea of employment. Ability to demonstrate the highest level of customer service and response. Demonstrated commitment to quality improvement and evidenced based practice.
