Primary Duties and Responsibilities: Coordinate all functions of the Palliative Care Program Work extensively with all stakeholders to build the market and grow the Program Develop relationships with referral partners in the community Focus on patients and their goals of care.

2 Work with the medical/clinical team to reducehospital readmissions Integral role and active participant in PCP interdisciplinary team Work closely in connecting patients and families with services to meet their needs Strong communication skills with a focus on challenging conversations Responsible for data analysis to help support and determine strategic programdevelopmentQualifications: Minimum Bachelor's Degree in Social Work, Health Care Administration,Nursing or Psychology.
