This situation can be solved by introducing Advanced Practice nurses, who could, for example, take on the responsibility of providing care to chronically ill people. This fact has been recognized by some authors, and reference was made to nurses with special training - taking on some of the trifle or Bagatelle diseases as a possible solution to the shortage. To date, the legal situation in Germany is quite clear on the concept of Nurse Prescribing . No nurse may prescribe any form of medical treatment. Nurse prescribing, however, is already standard Practice in many countries (The UK, Australia, New Zealand, etc.)

9 , and the legal situation in Germany has to change before any tasks in this field can be substituted (not delegated) to nurses. This position paper (Policy document) deals with developing the role of the Advanced Prac-tice Nurse, and his/her role in providing high quality care to patients in various settings. These settings may be in hospitals and not necessarily in rural areas only. However, this professional group of nurses could quite easily also take on the care for people living in areas with an under supply of medical doctors. It is important to recognise the contribution of the Advanced Practice Nurse to the health care system. Advance Practice Nurses all over the world have taken on certain functions which previously fell under the domain of the medical profession, such as diagnostic procedures (Gastro-Enteroscopy or breast screening, for instance), but they also took on specific thera-peutic measures, especially in Primary Health Care and mental health services.

10 Group therapy, counselling and case management, as well as drug rehabilitation has been part of the normal Nursing interventions within the domain of the Advanced Practice Nurse for more than 10 years. The main aim of the Advanced Practice Nurse is therefore providing expert direct care to the patient and not merely orchestrating the services at managerial level only.
