1 MinuteClinic Corporate Offices | One CVS Drive | woonsocket , RI 02895 | Pre-Employment Immunization and TB Services Employer CVS Health Minute Clinic Certiphi Customer Number Required Services / Immunizations: These Immunizations are required to be completed and/or documentation must be submitted by your start date. Please be sure to schedule the following Services if you do not have evidence/documentation of the following: Negative 2 Step TST: If you have had a negative PPD in the past year, this does not satisfy the requirement. You must complete a 2-step PPD. A chest x-ray is only needed if you have had a past positive TST. You must submit interpretation and symptom review completed by a physician. If you cannot have 2-step TST due to medical exemption and/or you decline/no proof of past chest x-ray: You have the option to schedule the T-Spot or Quantiferon test. MMR: (series or 2 shot vaccines or antibody titer) If you have documentation of the series of 2 shot vaccines or a positive antibody titer, you do not need to schedule this service.
2 You must provide copies of documents to Certiphi. A childhood history of disease DOES NOT satisfy the requirement. TDAP Vaccine: You do not need to schedule this service IF you have proof of vaccination with in 10 years. Documentation of only TD (tetanus) vaccine DOES NOT satisfy the requirement. Recommended Services / Immunizations: It is strongly recommended that you either order the following Services or provide evidence of immunity for the following immunizations. However, it is acceptable for you to decline these Services . If you are declining due to a medical reason, you must include a note from your PCP. Varicella: ( 2 shot series or antibody titer): Submit all evidence/proof of immunity. A childhood history of the disease does not satisfy as evidence of immunity. Influenza: ( Seasonal Vaccine) Submit evidence of vaccine within the year. Hepatits B:(3 shot series or antibody titer) If declining this service, you MUST sign Hep B declination form and submit to Certiphi.
