Of course there are certain analyses that require a longer time period and these will be handled on an individual basis.

6 The Turnaround Time for samples received after 3:00 pm shall begin the following business day at 8:00 am. RUSH FEES (When possible). Next BUSINESS DAY TAT 150% SURCHARGE. 2 BUSINESS DAYS TAT 100% SURCHARGE. 3 BUSINESS DAYS TAT - 75% SURCHARGE. 4 BUSINESS DAYS TAT 50% SURCHARGE. 5 BUSINESS DAYS TAT 25% SURCHARGE. SAME BUSINESS DAY TAT (When possible) - 300% SURCHARGE. WEEKEND TAT (When possible) - 400% SURCHARGE. SAMPLE CUSTODY AND STORAGE. To ensure correct receipt and handling of your sample, please forward a chain-of-custody form containing the following information: Full name, address, phone number of the person to whom the report and invoice are to be mailed. Purchase order number, if applicable. Complete details of sample identity and Analysis required. All Chain of Custody forms MUST be signed. Please provide adequate sample volumes for all requested analyses; if you are in doubt, call the lab.

7 SAMPLE CONTAINERS. San Antonio Testing will provide sample containers, upon request, at no extra charge. The client will pay shipping costs, if any. SAMPLE DISPOSAL. After issuing the final report, the remaining portions of the samples will be stored for 30 days and then disposed of. Sample can be kept for longer than 30 days at customer's specific request. The maximum storage by SATL shall not exceed 6 months. REPORTS AND INVOICE. The report will be e-mailed, (please provide email address). A hard-copy of the report will only be mailed if specifically requested by client. SAMPLING AND CONSULTATION. To have a technician collect the sample, the charge will be $ per hour, with a minimum of two hours, to be assessed. If a client asks San Antonio Testing to pick-up a sample, the charge will be $ per pick-up within city limits. 4 of 5. Price LIST EFFECTIVE JANUARY 1ST 2018.

8 USE OF RESULTS. Unless otherwise specifically agreed to by San Antonio Testing,LLC and the client, the information and analyses provided to the client shall be for the sole use of the client. TERMS OF PAYMENT. Payment is expected at time of sample drop-off, unless an account has been set up in which case terms will be net 30 days . All past due invoices are subject to a finance charge of per month. OFFICE HOURS. Monday through Friday, 08:00 to 5:30 . After hour sample drop-off available by prior appointment.
