I certify the above cart is in good operating condition and meets all requirements as described below (and as found in the HSVPOA Rules and Regulations: PRIVATE Cart/Neighbor Electric Vehicle (NEV) Regulations If Operated on Village roads and/or trails, each PRIVATE golf / NEV cart shall be equipped with: o Two (2) mounted headlights (white) and two (2) rear mounted taillights o A rear view mirror o Turf tires with a minimum size of 7 inches in width golf Carts / NEVs operated on Village golf course fairways shall not exceed a maximum gross operating weight of 1,749 pounds.)

2 golf Carts / NEVs having a maximum operations gross weight of 1,750 lbs. to 2,499 lbs. will be restricted to golf paths only. golf Carts / NEVs having a maximum operating gross weight of 2,500 pounds or above are prohibited from operating on any Village golf course. golf carts / NEVs operating on Village golf courses are restricted to no more than two (2) people (operator and passenger) golf carts / NEVs are limited to a maximum vehicle speed of 15 mph on all golf courses. When operating on Village golf courses, NEVs that do not have a selectable golf course speed control switch are restricted to golf paths only. Operating Regulations golf carts operating on Village roads shall: o Have headlights and taillights in good working order and turned on when operating on Village roads / trails. o Give way to trucks, automobiles, and pedestrians. o Shall not be operated on roads or sections of roads that have been designated with signs as golf Carts Prohibited
