1 Proposed Transition Program (LPN to ADN) for HCC. HCC s Transition Program is designed for licensed practical nurses who desire to enter the associate degree Program with advanced standing. Applicants must meet the admission requirements outlined for entry into the Transition Program . Applicants who meet the admission requirements outlined below and successfully complete the Nursing Transition Course (NUR 214) with a C or better will be allowed to enroll in NUR 211 and complete the Associate Degree Program requirements. The Program provides knowledge, skills and strategies to integrate safety and quality into nursing care.

2 It will allow students to practice in a dynamic environment and to meet individual needs which impact health, quality of life and achievement of potential. The Transition Program is a three semester Program of study which leads to an associate in applied science. Course work includes and builds upon the domains of healthcare, nursing practice and the holistic individual. The content will emphasize the nurse as a member of the interdisciplinary team providing safe, individualized care while employing evidence based practice, quality improvement and informatics. Halifax Community College is approved by the North Carolina Board of Nursing to offer the Practical Nursing Diploma, Associate Degree in Nursing and the LPN to RN Transition .

3 students from a community college outside of North Carolina or from the university system are not eligible for the Transition Program due to differences in course content across systems. The registrar will review general education courses from these colleges, and may award transfer credit for these. Criminal Background and Drug Screen Policy Criminal background checks and drug screens are required by our main clinical facility, Halifax Regional Medical Center, prior to students attending any clinical experiences at that agency, and are not part of the admissions process for HCC.

4 students who are unable to attend clinical due to the results of either the background check or drug screen will not be allowed to continue in the nursing Program . The decision on whether students may attend clinical is determined solely by the clinical agency. Alternate clinical assignments pose an ethical dilemma and will not be an option for students who cannot attend clinical due to criminal background or drug screening issues. The decision of the healthcare facility is final. Admission Procedure Qualification Deadline February 5, 2018. students are admitted to the HCC Nursing Programs in accordance with policies approved by the HCC.

5 President and Board of Trustees. HCC does not guarantee admission to all students who apply to the nursing programs. Completion of a nursing curriculum does not guarantee success in passing the licensure exam. Nursing students must be admitted to a nursing Program in order to take courses for credit in any nursing (NUR) course. Curriculum courses other than NUR courses may be taken before admission in the Program or after admission in the sequence offered. The following courses must be taken and credit received before admission into the Transition Program : BIO 168/169 (completed within five (5) years prior to acceptance into the Program ), PSY 150, and ENG 111.

6 *If the applicant has previous credit for BIO 168 & 169 that is older than 5 years, they may take BIO. 168 & 169 concurrently with the Transition courses. Admission to the nursing programs is limited. The deadline to complete the application process is February 5, 2018. HCC may contact students by mail, telephone, and/or e-mail during any step of the application process and encourages students to update the admission's department if any of this information changes. It is the responsibility of the applicant to ensure all requirements are met by the established deadline and to ensure accurate information is on file with the admissions office.

7 Application Process (Must be complete by February 5, 2018 at 4:00 pm). an application for admission to the college indicating LPN Transition Program . official records of high school graduation or equivalency and all post- secondary transcripts. a copy of signed Social Security card to admissions department. requirements for DRE 098, DMA 010, DMA 020, DMA 030, DMA 040, and DMA 050 or the equivalent course(s) with a Pass . the Health Sciences Admission Form. an overall GPA of or higher. GPA is determined by most recent college transcript with a minimum of 12 hours of non- developmental credit.

8 If student has no transcript with 12 or more credit hours, then high school GPA is used to determine eligibility. to admission the student must have taken and received credit for BIO 168/169 (completed within five (5) years prior to acceptance into the Program ), PSY 150, and ENG 111. *If the applicant has previous credit for BIO 168 & 169 that is older than 5 years, they may take BIO 168 & 169 concurrently with the Transition courses. must have and maintain an unencumbered LPN license. 9. Applicant must show proof of employment as an LPN who has completed a minimum of 1040 hours within the last year by completing the 1040 hours certification form.

9 Nursing Admission Testing Once the Application Process (Steps 1-9 above) is complete, students will be placed on a list for the testing coordinator, which qualifies the student to take the HESI Admission Assessment (A2) Exam. HESI A2 exam may be taken once per admission cycle (test dates will be posted on the Health Sciences webpage) and must be taken at Halifax Community College. HESI A2 exam at HCC will consist of the following sections: 1)Math 2)Reading Comprehension 3)Grammar 4)Vocabulary 5)Science A&P, Biology, Chemistry fee for the HESI A2 (currently $44, subject to change) must be paid at the HCC cashier's window and the receipt presented to the Testing Coordinator along with a valid picture ID before the test may be taken.

10 1) students will register for a specific date/time to take the HESI A2 after paying this fee. The tentative date for testing is February 1st , 2018 at 9am. 2)Instructions for registration will be provided at time of payment. tests (Career Readiness Certification, TEAS) are no longer valid as admission tests for Halifax Community College's Nursing Programs. cumulative score on the HESI A2 will be incorporated into the ranking process for the Transition Program . If the cumulative average is 70% or higher, the student will be eligible to be accepted into the Transition Program .
