EXHIBIT A General PROJECT INFORMATION The Tree House Humane Society is requesting proposals for a General Contractor for the construction of its new building including build out of the interior space.

2 Selection will be based on General conditions / fee PROPOSAL and qualifications per the requirements laid out herein for an open book guaranteed maximum price (GMP) project delivery. A. PROJECT DESCRIPTION OWNER: Tree House Humane Society 1212 West Carmen Avenue Chicago, IL 60640 ARCHITECT: The Dobbins Group 11 East Illinois Street, Chicago, IL 60611 312-321-0400 Mission Statement: Tree House is a humane organization that promotes the inherent value of every animal and strives to educate the public about proper and responsible animal care, with a focus on the care and placement of stray cats with special physical and emotional needs. We are committed to finding every healthy and treatable animal a home and to helping lead the way to a No-Kill nation. Tree House s New Adoption Center and Low-Cost Clinic Tree House is planning to build a state-of-the-art, environmentally-friendly Adoption Center and low-cost Veterinary Clinic in Chicago, which will be approximately 15,000 square feet.

3 Our focus will remain on rescuing and rehabilitating sick and injured stray cats, educating the public in responsible cat and dog care and behavior, and providing guidance and direct support for the care of feral cat colonies living outside. Adoption Center The cat housing areas will be cageless, with rooms of varying sizes to accommodate single cats or a colony of 10 cats. Each room will be designed with the ultimate health and comfort of the cats in mind and will feature multi-tiered shelving platforms and comfortable furniture that can be easily sanitized. The ventilation system will offer multiple air exchanges every hour and will feature an ultraviolet filtering system to destroy air borne bacteria and viruses. Sunlight will be abundant in all housing areas and outdoor enclosures will be accessible in most of the adoption areas. Pet Food Pantry & Supply Store The Pantry will provide food and supplies (litter, toys, treats) free of charge to support the pets of low-income families and individuals.

4 This program will be subsidized by a Supply Store selling pet food and merchandise to adopters and the General public. August 23, 2014 Page 4 Tree House Humane Society New Veterinary Care and Cat Shelter Education Center/Training Area The education/training area will feature interactive and educational exhibits, training sessions on cat and kitten care and behavior, training and certification classes for dogs, and informational seminars on topics like Pet Trusts, Pet Diet and Nutrition, and Trap-Neuter-Return (TNR). B. SUMMARY OF WORK A. Project Identification is as follows: 1. Location: 7225 North Western Avenue, Chicago, IL 60645 2. Owner: Tree House Humane Society B. Contract Documents, dated June 16, 2014 prepared by The Dobbins Group. C. The Work covered by these Contract Documents generally consists of the construction of a new approximately 14,815 sq. ft. cat shelter and outpatient animal clinic, and as described in the drawings and these Contract Documents.

5 The following shall be included in The Bid: 1. All labor provided for the Project shall be UNION. 2. Based on the anticipated start date and The Contractor s schedule, accommodations for winter conditions shall be part of the work only as required. Please see requested additional information for further instructions. 3. The General Contractor shall coordinate all required inspections throughout the duration of the project. 4. Work in the Public Way shall confirm to all rules and regulations of The City of Chicago and each agency having jurisdiction over required above or below grade improvements and utility connections. 5. The General Contractor shall coordinate all Public Utility work required for the project. 6. The Owner anticipates authorization to commence The Work immediately upon receipt of a building Permit allowing construction of the building and site. Drawings have been submitted to the city for technical reviews, and permit approval is anticipated by October 15, 2014.

6 7. Earth Retention Systems requirements are generally set forth in these Contract Documents, but are still in review and subject to approval by the Office of Underground Coordination. August 23, 2014 Page 5 Tree House Humane Society New Veterinary Care and Cat Shelter D. Work provided by the Owner generally consists of following: 1. Specialty veterinary equipment, cabinets, and cages 2. Specialty Feline housing accessories 3. Contract furnishings 4. Appliances (GC to install) and vending machines 5. Computer and networking equipment 6. Security systems 7. Telecommunications systems 8. Audio / Visual Systems and components 9. Specialty signage and wall Graphics not specifically code required and noted in the Contract Documents. The General Contractor shall be responsible for coordination of the above items and connections to portions of the work as noted in the Contract Documents. C. PROJECT SCHEDULE The following is the anticipated project schedule as it relates to the General Contractor .

7 It is our intention to select the Contractor to aid the team with pre-construction services. Additionally, they will pursue the permit process as early as is feasible. EXHIBIT B PROPOSAL GUIDELINES Contractors must respond to all items in the order and format presented in this document. Each page and point should be numbered in sequence corresponding to the page and sequence number appearing herein. Each section should start a new page. Please provide as much pertinent information as possible. Please include: A. Title Page Identify the name, address, e-mail and telephone number of Contractor and of Contractor s representative who will negotiate the contract. B. Executive Overview Identify why your company is best suited to handle the construction required. Identify any additional or value-added service that your company can supply to Owner. C. The service description for each Phase in the format described herein. D. The formal PROPOSAL /bid response must be received no later than 4:00 September 3, 2014.

9 Please provide one (1) copy of the PROPOSAL to each of the following. Contractor 's PROPOSAL shall, at a minimum, comment and address the following issues: A. FIRM INFORMATION AN PROJEC TEAM 1. Brief overview of your firm's history, from the firm s inception through its current operations. Comment on key officers of the company including backgrounds and responsibilities, locations of other offices and the number of professionals employed, etc. 2. Provide a current Contractor s Qualification Statement, AIA Document A305 or Certification that indicates qualifications relative to industry standards. Provide detailed resumes of the proposed individuals who will be permanently or partially assigned to the proposed project.

10 Discuss percentage of time each individual will be assigned to the project and whether their location would be on site or off site. For the individuals assigned directly to the project, please identify any other projects that the individual is participating in during the term of the project and their commitment of time to those projects and the anticipated project schedules. B. PROJECT GMP APPROACH 4. Summarize your approach to budgeting and purchasing subcontractors and material trade items. Provide sample budget formats and reports of documentation your firm utilizes in making awards. Comment on procedures your firm utilizes in making subcontractor award recommendations. 5. Provide an explanation as to the method you propose to assemble the Guaranteed Maximum Price (GMP) and define your philosophy to the following terms as it pertains to the GMP: Allowances Contingencies Line Item Savings Overall Savings Scope Changes Shared Savings 6.
