3 Invoicing Instructions 1 Information required on invoices for payment 2 All invoices shall comply with Section 20 of the VAT Act No 89 of 1991. 3 All invoices should be forwarded to copy 4 Invoice enquiries must be forwarded to 012 301 5633 Requirement list SPECIFICATIONS: AgriSeta requires the service of suitably qualified landscape Gardening and Ground Maintenance company to maintain trees, hedges, flowerbeds, shrubs beds, plant containers, cleaning windows and balconies on the AgriSeta premises for a period of 12 months, with an option to renew until March 2020. Specification of Requirements: Gardening SERVICES The work will involve all gardening and garden maintenance SERVICES including hedge trimming and planting of flowers/shrub beds, weeding, watering, feeding of plant beds, removal of old flowers, application of weed killer, garden waste disposal and.

4 The SERVICES provider will be required to be on site twice a month and on Mondays for gardening. Capacity of Service Provider The Service Provider must be an established landscape gardening SERVICES provider with appropriately qualified and experienced staff capable of providing the SERVICES required.

5 Insurance/ Responsibility for injury to Contractor s staff or Third parties The Service provider must provide evidence of appropriate employer s and public liability insurance to cover the risk of injury to the contractor s staff and / or any third party such as staff of the AgriSeta or members of the public who may be accidentally during the course of gardening operations. 1 EVALUATION PROCESS AND CRITERIA COMPLIANCE WITH MINIMUM REQUIREMENTS All bids duly lodged as specified in the REQUEST for Bid will be examined to determine compliance with quote requirements and conditions. Bids with obvious deviations from the requirements/conditions will be eliminated from further consideration. DETERMINATION OF SCORE FOR FUNCTIONALITY The evaluation criteria and weights for functionality as indicated in the table in below, will apply.

6 2 CRITERIA MAXIMUM TO BE AWARDED 1. Functionality 100 Experience of prospective contractor 50 Relevant experience to provide gardening & cleaning SERVICES Minimum three experience in providing similar service. 3 years experience = 50 points Less than 3 years= 0 points References 50 Detail of similar service delivery, testimonials and contactable reference from 3 clients for the company proposing 3 testimonials and 3 contactable reference submitted = 50 points 1-2 testimonials and contactable references submitted= 20 points The score for functionality should be calculated as follows: Reference No: AS/2018/RFQ/011/Cleaning & Gardening SERVICES Service:: Gardening and Cleaning Page 4 Each panel member shall award values for each individual criterion on a score sheet. The value scored for each criterion shall be multiplied with the specified weighting for the relevant criterion to obtain the marks scored for the various criteria.

7 These marks should be added to obtain the total score for functionality. The score of each panel member shall be added together and divided by the number of panel members to establish the average score obtained by each individual respondent for functionality. If a quotation fails to score a minimum of 70% out of 100% for functionality, his quotes will be disregarded from further consideration. meet the minimum threshold for functionality as per paragraph above. 3. CALCULATION OF POINTS FOR PRICE If appropriate, implied contract price adjustments will be made to the cost proposals of all remaining bids. The lowest acceptable quote/proposal (adjusted or not), will obtain the maximum score allocated for price, taking into consideration the relative competitiveness of the proposed price.

8 The other quotations/proposals with higher prices (adjusted or not), will proportionately obtain lower scores. The price points scored out of 80 shall be calculated according to the following formula: The 80/20 preference point system (from R30 up to R50 million budget) )minmin1(80 PPPtPs where Ps = points scored for price of the quote/proposal under consideration Pt = rand value of quote under consideration Pmin = Rand value of lowest acceptable quote 4. COMBINING PRICE AND B BBEE STATUS LEVEL POINTS All remaining quotations will be evaluated as follows: 80 Marks will be awarded for price and 20 marks will be awarded for the B-BBEE status level certificate submitted. If no certificate is submitted, zero (0) points will be awarded. 9 5. ADJUDICATION OF QUOTE The relevant award structure will consider the recommendations and make the final award. The successful respondent will usually be the service provider scoring the highest number of points or it may be a lower scoring quote on justifiable grounds or no award at all. 10 If an explanatory note is provided, the paragraph reference must be indicated in a supporting appendix to the application submission. Should respondents fail to indicate agreement/compliance or otherwise, AgriSETA will assume that the respondent is in compliance or agreement with the statement(s) as specified in this quotation . Quotess not completed in this manner may be considered incomplete and rejected. AgriSETA shall not be liable for any expense incurred by the respondent in the preparation and submission of a quote. 1 FRAUD AND CORRUPTION All providers are to take note of the implications of contravening the Prevention and Combating of Corrupt Activities Act, Act No 12 of 2004 and any other Act applicable. 2 GENERAL CONDITIONS OF CONTRACT The General Conditions of Contract must be accepted.
