After reviewing the Federal TEACH GRANT website and reading the conditions of AWARD and eligibility requirements, I am requesting further CONSIDERATION for the Federal TEACH GRANT . (All of the following requirements must be met). I have completed the Free Application for Federal Student Aid (FAFSA). You do not have to demonstrate financial need for this GRANT . I am a citizen or eligible non-citizen I am enrolled as an undergraduate, working on my first degree at EIU, in one of the following high need fields (please check one): BS in Ed in Special Education BA in Foreign Languages: French, German, or Spanish with Teacher Certification BA in Mathematics with Teacher Certification BS in Science with Teacher Certification I meet one of the following academic achievement requirements (check all that apply and attach any requested documentation as noted below): I scored above the 75th percentile on my ACT test in at least one subject/battery (English, Reading, Math, or Science).
2 You must attach a copy of your ACT percentiles; or I graduated from high school with a GPA or above; or I have established a overall GPA or higher in college at EIU or another institution. (Please note that transfer GPA can only be used for eligibility at initital term of transfer.) Please provide name of transfer institution: I understand if I am awarded the Federal TEACH GRANT , I must do the following before my Federal TEACH GRANT will disburse to my student account: 1. Annually sign a TEACH GRANT Agreement to Serve. 2. Annually complete entrance counseling related to the Federal TEACH GRANT 3. Maintain a EIU cumulative GPA. (This requirement is waived for those who scored above the 75th percentile in at least one subject on the ACT test.).
