The trainings are one part of the work being done to reduce recidivism statewide through the Department of Correction s Statewide Recidivism Reduction Initiative (SRR).

3 The goal of these trainings is to develop strong connections among the community, prisons and community-based corrections, the community Mental Health and Disability Services system, and the substance-use disorder service systems for individuals reentering the community after incarceration. We hope these connections will lead to a greater understanding of the resources available to individuals to help them establish successful and permanent lives in the community, to reduce recidivism, and to increase public safety. Trainings will be conducted three times in each of Iowa s eight judicial districts over the course of a year beginning in February 2016. Training Objectives 1. Develop an understanding of the challenges faced by individuals and their families as they reenter the community after incarceration.

4 2. Learn how the corrections and community service systems operate, including information about Services and supports available and how they interact to support persons reentering the community. 3. Establish connections between the corrections and community Services systems, including individual and agency contacts. Resource Guide This Resource Guide has been developed to compliment the information shared at the training, and serve as an ongoing Resource to help connect individuals and families with necessary Services and support to help them maintain stability, and find success in the community after incarceration. The goal of this Guide is to provide simple overviews of the many Services and supports available in the community, and where to go to make referrals or get more information.

5 We plan for this Guide to grow over the course of the project and will make updated electronic versions available to individuals who have attended past trainings and their organizations as information is added. Please contact Caitlin Owens if you have feedback on what would make this Guide more useful, or would like to submit information about Services , supports, or other resources available in your community that help individuals and families in the community after incarceration. 3 Mental Health and Disability Services Regions Overview of Mental Health and Disability Services Regions: Mental Health and Disability Services (MHDS) regions provide an array of Services to individuals who are not eligible for Medicaid but have needs related to Mental Health or Disability status, and individuals who are enrolled in Medicaid but need access to Mental Health or Disability Services not covered by Medicaid.

6 All regions must provide a set of core Services to individuals with a Mental illness and/or an intellectual Disability at or below 150% of federal poverty level (FPL). Many regions provide Services to individuals with a developmental Disability or brain injury, and individuals who earn between 150-200% of FPL. All regions fund Services beyond the required core Services . There are currently 14 approved MHDS Regions operating in Iowa. Regions are made up of three or more counties, with the exception of Polk county which operates as a one county region. Individuals to Connect: Those who are not Medicaid eligible but have needs related to their Mental Health or Disability diagnosis Those who are enrolled in Medicaid but need access to Services not covered by Medicaid Eligibility: Funding available to support individuals beyond the required core population ( Mental illness and intellectual Disability ) varies by region.

7 Income guidelines and other eligibility criteria vary as well. Contacting a regional office directly is the best way to get more information on eligibility. How to Contact/Refer for Services : Regional Offices: Most regions have multiple offices located within the region where individuals seeking funding for Mental Health and/or Disability Services can fill out an application for Services . Local regional offices are a good Resource for anyone who has questions about Services , eligibility, or the application process. Access Points: Regions are required to have additional access points around the region. These are usually housed within community service organizations, hospitals, or other places where individuals access Mental Health and/or Disability Services . Applications are available at designated access points, and staff have been trained to assist with the application process and forward completed applications to the region.

8 More information about each region, including contact information, begins on page 5. 4 Core Services Core Services are the Services and supports regions are required to provide to eligible individuals. Treatment Services , such as: o Assessment and evaluation o Mental Health outpatient therapy o Medication prescribing and management o Mental Health inpatient treatment Employment support, such as: o Day habilitation o Job development o Supported employment o Prevocational Services Basic crisis response, such as: o 24 hour access to crisis response o Evaluation o Personal emergency response system Recovery Services , such as: o Family support o Peer support Support for community living, such as: o Home Health aide o Home and vehicle modification o Respite o Supportive community living Service coordination, including coordinating physical Health and primary care, such as.
