A photocopy of the return confirmed travel ticket. 2 Proof of Adequate Finance(At Least One) For Individual tourist Receipt of currency exchange of USD 700/ 20,000 thai Baht per person OR USD 1400/ 40,000 thai Baht per family Baht. A photocopy of the valid international credit card with applicant s name visible.
2 Bank statement of six months with current balance of USD 700 per person or USD 1400 per family. For Incentive Group Letter of Financial Guarantee from Company for USD 700/ 20,000 thai Baht per person 3 Accommodation Proof Of Hotel / accommodation reservation in Thailand with contact number and address If intended to stay with relatives or friends, must provide invitation letter along with copy of their ID / passport. In case they are not thai , must provide their work permit, valid at least six months. 4 tourist visa (MEDICAL TREATMENT) Same requirements as above from point one to three 5 ADDITIONAL DOCUMENTS Medical report from local hospital requesting treatment in Thailand Acceptance letter from Thailand s hospital for treatment of applicant REMARKS Please Note Validity of the visa denotes the time period during which the traveller can use the visa .
