NOTICE TO CCW PERMIT APPLICANTS On October 9, 1986, the California Supreme Court issued its opinion in a Civil Action: CBS, INC. VS. SHERMAN BLOCK (9186) 42 646. CBS sued the Los Angeles COUNTY Sheriff under the Public Records Act for information that appears on License applications and licenses on file.

5 The court issued a broad opinion holding that information on both the application and in the license itself is open to public inspection under the Public Records Act (Government Code, Section 6250 et. Seq.). The opinion discussed disclosing both the identity of the license holder and the reason the license was issued. Other information from the license/permit will be released unless the release of such information would endanger the licensee. This notice is to advise you that in the event of such a request to the San JOAQUIN COUNTY Sheriff s Office , we will release such information as required by the Supreme Court decision. Information contained in your file will remain on file with us for a two (2) year period after date of expiration and will remain Public Record for that time.

6 Should this Office receive a request for information relating to your Permit/Application, we will advise you in writing. At that time, it will become your responsibility, should you object to the information being released, to obtain an injunction in court to stop the release mandated by law. If an extension is applicable, another ten days may be allotted, however, the requestor must be advised of the extension. It will be your responsibility to notify this Office of your intent to file for an injunction with the court. Please file this letter with your important documents and refer to it should it be necessary for this Office to notify you of such a request. Appendix A Initial this page to indicate you have read this notice _ Rev: 070117 4 NEW LICENSE FIREARM TRAINING REQUIREMENTS We require that all new CCW applicants attend a 16-hour training course, which shall include the following: 1.

7 Instruction on Firearm Safety 2. Weapons maintenance and inspection 3. Actual demonstration of firearm skills with each weapon that is to be listed on the CCW Permit. 4. California laws regarding the possession and permissible use of firearms 5. Use of force 6. ALL applicants must demonstrate that they can: a. Qualify on a certified range with each individual weapon that will be listed on their CCW Permit (maximum of 3 weapons). b. The make, model and serial number of each weapon must be listed on the certificate, and must be verified, signed and dated by the firearm instructor. Failure to successfully pass the course of instruction, or the firearm qualification course, upon remediation, shall constitute failure and termination of the applicant process RENEWAL LICENSE FIREARM TRAINING REQUIREMENTS We require all CCW renewal applicants attend a minimum 4-hour training course, which shall include the following update: 1.

8 Firearm safety. 2. California laws regarding the possession and permissible use of firearms, including Use of Force. ALL applicants must demonstrate that they can: 1. Qualify on a certified range with each individual weapon that will be listed on their CCW license (maximum of 3 weapons). 2. Each weapon is to be inspected by the firearms instructor. 3. The make, model and serial number of each weapon must be listed on the certificate. 4. The firearm instructor shall sign and date the training certificate, verifying the applicant has met these minimum requirements. Failure to successfully pass the course of instruction, or the firearm qualification course, upon remediation, shall constitute failure and termination of the applicant process.

9 Appendix B Initial this page to indicate that you have read requirements Rev: 070117 5 HOLD HARMLESS AND INDEMNIFICATION AGREEMENT As a condition of receiving a permit to carry a concealed firearm from the San JOAQUIN COUNTY Sheriff s Office , I , agree to hold (Please Print Name) the San JOAQUIN COUNTY Sheriff and his agents harmless from and to indemnify them against any and all liabilities, including judgments, costs and attorney s fees, that may occur as the result of my possession and/or use of the concealed firearm(s) named on the license issued to me by the San JOAQUIN COUNTY Sheriff s Office . Permit holder agrees that by accepting the Permit to Carry A Concealed Weapon, under the provisions of PC 26150, he/she will hold harmless the COUNTY of San JOAQUIN , San JOAQUIN COUNTY Sheriff s Office and any and all officers or employees thereof from any claim, suit, or judgment arising in any way to the exercises of the privileges granted by any Concealed Weapon Permit issued to this applicant.

Additional Restrictions: CCW License holders may not carry weapon during work hours or during transportation of children in the capacity as licensed Family Day Care Provider per the California Department of Social Services, Community Care Licensing Division (CCLD), Title 22, and Regulation #102417. Firearms must be unloaded and locked during work hours and/or when children present. All ammunition must be kept separate from weapon and locked.
