End-of-Season coach and Assistant coach Evaluation Form The SANDWICH SOCCER club (the SSC) is committed to providing all SSC players with volunteer coaches, assistant coaches, and team managers who are always eager to provide their time, enthusiasm, and experience in the sport of SOCCER (and other team sports) for the benefit of our many players. We seek coaches who will provide their players, regardless of player age, with opportunities for individual technical skill development, improved team play, increased fitness and strength, a greater understanding of the game, and increased confidence, while providing these opportunities in an encouraging, constructive, safe, competitive, and always enjoyable environment. In order to ensure that we are providing our players with coaches that are providing these opportunities, we ask parents and players every year following completion of the season to provide and evaluation of their respective coaches (both head coach and assistant coach ).

2 The SSC Board members use the information in the evaluations we receive from you during our yearly selection and approval of both age group directors and coaches for the next season. We request that you be a part of this important process by completing this evaluation form and returning it to the SSC. All information you return can be submitted anonymously and will be confidential. Thank you in advance for your responses! _____.
